•** **Behavioral Health Registered Nurse – Home Care
$5,000 Sign-On Bonus!
**Territory:** Chicopee, Holyoke, Springfield
**Schedule:** Monday–Friday, Days

•** **What You’ll Do
+ Provide compassionate, holistic care for behavioral health patients in their home setting
+ Assess and manage a caseload to identify physical, psychosocial, and environmental needs
+ Develop and adjust individualized care plans in collaboration with physicians and interdisciplinary team members
+ Educate patients, families, and facility staff on behavioral health and symptom management
+ Supervise and support LPNs and behavioral health aides to ensure quality of care
•** **What You’ll Need
+ Current, unrestricted **RN license** in the state of practice
+ Minimum **1 year of clinical nursing experience**
+ Reliable **vehicle, valid driver’s license, and auto insurance**
+ Willingness to **travel within assigned territory (approx. 50%)**
+ **Associate or Bachelor’s degree** in Nursing (preferred)
+ **Home Health or Hospice experience** a plus
+ Experience with **Homecare HomeBase** preferred
