Intensive Case Manager

The City of Bangor has a new career opportunity for an Intensive Case Manager within the Public Health and Community Services Department. The Intensive Case Manager (CM) coordinates care to individuals living with HIV/AIDS. The CM provides a range of client-centered services to link clients with health care, psychosocial, substance abuse, and other community services. Also providing support around timely medical treatment and coordination of continuity of care. The goal with each client is to obtain viral suppression through a multi-team approach. This position is a full-time (37.5 hours per week) with a comprehensive benefits package. Highlights include:

This position’s full salary range is $29.14 – $32.92 per hour and is nonexempt.
