    Job Description

    A leading home healthcare provider is seeking a Clinical Nurse Manager to join their New York region. This is a full-time position offering a balanced mix of fieldwork and office-based responsibilities.

    • Fieldwork (40%)

     

    o Conduct monthly visits for each case (caseload ranges from 15 to 30 clients)

     

    o Support admissions and perform new client assessments

     

    o Develop and update care plans

     

    o Supervise and mentor field staff

    o Territory includes northern Bronx, Kings, New York, Queens, Richmond

    • Office-Based Duties (60%)

     

    o Observe and validate field staff competencies (new employees and annuals)

     

    o Review and ensure accurate case load documentation

    o Lead staff training sessions and in-service education

    • 2 education days per month, the role is very case management-focused

    • Infants, children, & adult patients

    • 150 nurses in the New York region

    Skills and Requirements

    • 2 years of RN experience (Home health is preferred)

    • Excellent communication skills

    • Comfortable traveling via car or public transportation across all 5 boroughs

    • Comfortability with on-call once every three weeks • Previous management experience

    • Home health care experience highly preferred

    • NICU/PICU experience
