Licensed Practical Nurse – Home & Community
Part Time (0.80 FTE) | Day Shift | Florence, Oregon
Hourly Range: $28.02 – $42.04
The work schedule for this position includes every other weekend.
Bring your compassion and clinical skills to a role where your care truly makes a difference. **PeaceHealth** is seeking a dedicated **Licensed Practical Nurse (LPN)** to join our **Home & Community team** on the beautiful **Oregon Coast in Florence, Oregon** .
As an integral part of our Home Health care team, you’ll provide direct patient care in home settings under the supervision of an RN, helping patients heal and live safely and comfortably at home. This position offers the opportunity to deliver meaningful, one-on-one care while enjoying a supportive, mission-driven environment.
What You’ll Do:
+ Deliver skilled nursing care, including administering medications and treatments as prescribed.
+ Perform wound care, venipuncture, and manage PICC lines, ports, drains, and other devices.
+ Assess and report patient conditions, collaborating closely with the RN case manager and interdisciplinary team.
+ Educate patients and caregivers on managing chronic conditions, medications, and self-care techniques.
+ Document all care accurately and completely in compliance with regulatory and agency standards.
+ Ensure safe, timely, and compassionate care that supports each patient’s individualized plan.
What You Bring:
+ Graduate of an accredited **State Board of Nursing LPN program** .
+ Current **Oregon LPN license** and **Basic Life Support (BLS)** certification required.
+ Valid **driver’s license** and proof of auto insurance.
+ Minimum of **one year of clinical nursing experience** preferred, ideally in home health or community care.
+ Strong communication skills, professionalism, and a passion for serving others.
