**Primary Function/General Purpose of Position** _:_
_The Human Services Coordinator assists the case management team with the delivery of clinical case management services to assigned patient populations and,_ under the supervision of the Program Director, is responsible for providing supplemental program coordination and sound operational direction for the AccessHealth Tri-County Network program. _Services include but are not limited to outreach, monitoring, service planning, financial resource planning, case coordination, advocacy, education, and follow-up directly related to the patient._
**Essential Job Functions** _:_
+ _Serves as the point of contact for patients' care coordination throughout hospital departments and community partners._
+ _Acts as a patient safety advocate by participating in ongoing quality improvement in the department. Incorporates the professional practice model into care delivery._
+ _Meet regularly with the Community Care Navigation Team to discuss pending status and ongoing conditions of patients and provide guidance to Community Care Navigators as needed with medical and social patient care coordination issues and evaluate effectiveness._
+ _In collaboration with the Program Manager, provides guidance to teammates in the absence of the Program Director._
+ _Provide daily casework on assigned set of patients enrolled in the AccessHealth Tri-County Network Program to include completing applications, developing care plans, and other aspects of social service coordination._
+ _Assist Program Director in developing and maintaining a network of human services and community resources that partner with AHTN to provide supportive services to clients._
+ _Documents all findings, actions, and plans using the guidelines and standards of Access Health._
_Input data into patients' charts and assist with running data reports when needed._
+ Assist case managers with applications to benefits programs and reimbursement sources, including, but not limited to, Medicaid and Medicare, AIDS drug assistance applications, ACA Insurance marketplace plans, prescription assistance programs, charity applications, and other insurance applications.
+ Facilitate case coordination, which is non-duplicative and collaborative; be responsible for timely two-way communication between other agencies; and ensure continuous seamless case management.
+ Maintain strict confidentiality of all client data and observe all policies concerning the release of information.
+ Performs other job duties and responsibilities as required.
Licensing/Certification

Education
_Bachelor’s in Social Work, Human Services, Sociology, Psychology, Counseling and Health Education or related field (Required)_
Work Experience
+ 5 Years of experience in case management or counseling, preferably in a public health setting or with high-risk populations
+ Program management or coordination experience preferred.
+ Experience related to the marginalized population is required
+ Related unpaid and internship experience is accepted.
