    Job Summary: The Manager, Counseling Services is responsible for the coordination, planning, and day-to-day supervision of hospice clinical services provided by the social work and spiritual care staff in compliance with all regulatory standards and agency policies.

    Essential Functions:

    + Provides overall supervision of patient care and operational services provided by social work and spiritual care staff.

    + Evaluates clinical services through a variety of means such as home visits, conferences, and record reviews. Discusses, verifies and documents findings. Provides feedback to staff, ensuring that both effective performance and delivery of quality care are reinforced. Is available during normal business hours and when on call to guide and assist clinical staff as appropriate.

    + Assists clinical staff in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a Plan of Care.

    + Participates in Interdisciplinary Group meetings with Hospice staff to facilitate coordination of care and discussion assuring that a quality social work and spiritual care service is provided.

    + Participates in public relations and community activities that promote the Hospice organization’s role as an effective member of the Hospice and health care delivery system.

    + Develops ongoing positive and professional relationships with contracted providers and other related bodies including referral sources.

    + Participates in financial assistance process with Director of Finance or designee, for patient financial assistance applications.

    + Responsible for workflow in the electronic health record (EHR) associated with disciplines that are supervised.

    + Monitors and ensures adherence to agency standards for provision of care and that documentation and medical records comply with local, state and federal regulations.

    + Provides strong, visible leadership and direction through compelling communication of the agency vision, values, key strategic initiatives, and job expectations.

    + Creates an environment of accountability by constructively and appropriately challenging staff to deliver agreed upon outcomes/results and model desired values and behaviors.

    + Understands and complies with the Medicare Hospice Conditions of Participation.

    + With the AVP, Counseling Services and Integrative Therapies, responsible for the quality of services and quality initiatives impacting the clinical area and sustainment of clinical quality initiatives.

    + Coaches, mentors, and develops staff and holds them accountable to meet agency/supervisor expectations.

    + Responsible for creating a work environment that assists staff in completion of tasks in meeting strategic goals.

    + Monitors and ensures compliance with applicable local, federal, and state regulations, OSHA, infection control, and Quality Partners standards, compliance guidelines, privacy protections, and protected health information.

    + Provides oversight of responsible areas for the organization’s budget.

    + Hires new staff in accordance with established procedures. Orients or delegates orientation of new employees and ensures that employees have adequate ongoing training and development.

    + Establishes and communicates job responsibilities, performance standards, priorities and expectations to staff.

    + Routinely observes and documents both good and poor job performance and conducts timely performance evaluations according to set standards.

    + Ensures that staff is properly licensed and/or credentialed.

    + Informs and works in consultation with Human Resources on employee discipline issues (i.e., policy violations, performance issues) and terminations.

    + Upholds organizational policies and procedures and all regulatory and legal requirements.

    + Models the NorthStar standards to care for every person, every time, 100% of the time.

    Qualifications:

    + Graduate of a Master’s program in Social Work accredited by the Council on Social Work Education.

    + Maintains license/registration as a Master’s Social Worker (MSW) from the State of Michigan.

    + Two (2) to three (3) years’ clinical experience is required in healthcare, preferably in hospice, home health, or community-based care along with one (1) to two (2) years’ experience in leadership or supervisory role preferred.

    + Experience with palliative and quality end-of-life care preferred.

    + Experience working with multi-disciplinary teams, building consensus, and leading people and projects forward is preferred.

    + Must be an excellent communicator with the ability to bring out the best in others; able to set and complete strategic goals.

    + Must possess excellent interpersonal skills, sound judgment, effective organizational, prioritization and follow-through skills, attention to detail, tact, dependability, emotional intelligence and the ability to promote positive, constructive relationships with communication and collaboration at all levels with the ability to maintain confidentiality.

    + Has understanding of performance improvement and can communicate and operationalize performance improvement initiatives at the departmental and organizational level.

    + Ability to communicate effectively and professionally in writing and verbally to perform management duties and to promote good working relationships. Demonstrated experience in public speaking required.

    + Ability to effectively use technology in support of management and clinical operations.

    + Ability to prioritize multiple demands.

    + Proof of current tuberculin testing required; will not be allowed patient contact until tuberculin clearance is documented.

    + The physical and sensory demands of the position include: vision, effective speech and hearing for extensive telephone contact; repetitive motion; traveling; driving or riding in motor vehicle; standing, sitting, walking, bending, reaching, and stretching; lifting up to twenty-five (25) pounds unassisted.

    + Must have reliable transportation to be able to travel and maintain the rigors of a busy schedule. Frequently works variable hours/days; activities and workload may require extended days.

    + Must be eligible to work in the United States.
