    Position Overview:

    The CARE Program Supervisor plays a key leadership role within Community Programs, providing oversight, coaching, and support to a team of CARE Case Managers and a Peer Health Coordinator. This position ensures high-quality, client-centered service delivery by monitoring performance, facilitating professional development, and reinforcing consistent standards of care across the team. In addition to supervising direct service staff, the Supervisor also maintains a small caseload of complex cases and collaborates closely with internal and external partners to strengthen community support networks. This is a dynamic role that requires strong interpersonal, organizational, and leadership skills.

    Essential Duties and Responsibilities:

    + Provide daily supervision and support to CARE Case Managers and Peer Health Coordinator, including regular check-ins, performance feedback, and case consultations.

    + Serve as a resource for complex case management challenges, modeling trauma-informed, client-centered practices.

    + Monitor caseloads and service delivery outcomes to ensure quality and compliance with agency standards and funder requirements.

    + Facilitate onboarding, training, and continuous professional development for case management staff.

    + Review and approve client records for completeness, accuracy, and timeliness.

    + Lead regular team meetings to foster collaboration, consistency, and knowledge-sharing.

    + Maintain a small caseload of clients with high needs or particularly complex cases.

    + Collaborate with community partners, funders, and internal teams to enhance service coordination and access to resources.

    + Participate in budget meetings with fiscal while working close with supervisor and fiscal team on budget oversight

    + Participate in audits, reporting, and quality improvement initiatives as needed.

    + Uphold a culture of equity, accountability, and professional growth within the team.

    Required Education and Experience:

    + Bachelor’s degree in Social Work, Psychology, Human Services, or related field; Master’s degree preferred.

    + Minimum of 3 years of experience in case management, with at least 1 year in a supervisory or team lead role.

    + Demonstrated ability to coach and support staff in a fast-paced, client-focused environment.

    + Strong interpersonal, organizational, and conflict-resolution skills.

    + Proficiency in case documentation and use of case management software.

    + Knowledge of local community resources, public benefits, and support services.

    + Ability to manage priorities and adapt to changing client and program needs.

    + Experience working with diverse populations and commitment to health equity and cultural humility.

    + Bilingual in English and Spanish preferred.

    + Certification in case management or related field a plus.

    Compensation:

    A salary within the range of $30.24 to $40.91 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
