JOB DESCRIPTION JOB TITLE: Veteran Outpatient Case Manager DEPARTMENT: Adult Services REPORTING RELATIONSHIP: Adult Services Director HOURS: Full Time POSITION SUMMARY: The Veterans Outpatient Case Manager plays a crucial role in connecting veterans with the resources and services they need to thrive in their post-military lives. This position involves assessing the unique needs of veterans and their families, ensuring they receive appropriate support in areas such as healthcare, education, and employment. This position will work closely with the Veteran Program Coordinator and other agencies/organizations to facilitate access to benefits and services and advocating for the rights and needs of veterans. This position is also responsible for aiding veterans regarding eligibility for programs and benefits offered and/or facilitated by the Department of Veterans Affairs and providing program support to the Veterans and active-duty military personnel coordinate and network with multiple agencies and organizations with differing cultures, as well as an understanding of veterans' issues and benefits. Assist veterans and family members directly by providing information on how to obtain benefits and acquire and disseminate information on current issues of interest to veterans. Description of duties and responsibilities: * Conduct initial and annual assessments utilizing basic therapeutic methods, obtaining pertinent information for the purpose of formulating a diagnosis of psycho-social issues. * Assist with the development of behavioral health treatment plans for all veterans receiving behavioral health services. * Provide information and coordination of intensive, community-based behavioral health care; assist with providing resources regarding VHA health guidelines. * Inquires military service and assist with directing to care coordinating services through the agency. * Offers and aids with enrollment in the VHA for delivery of health and behavioral health services to person affirming former military service. * Provides coordination between the care of substance use disorders and other mental health conditions for veterans and active-duty military personnel. * Assist with integration and coordination of care for behavioral health conditions and other components of health care, coordinates recovery-oriented services that adhere to VHA recovery guidelines. Education: Master's in Social Work, Psychology, Nursing or a related field. Have experience in a veteran's services and understanding of the veterans' and military behavioral health needs. Prefer military experience. Requires a working knowledge of the theory and practice of mental health counseling and psychotherapy; the dynamics of human behavior of individuals and groups; structure and function of the mental health and welfare services; the legal aspects of counseling and psychotherapy; the DSM-V-TR and diagnostic terminology. Candidate must have a minimum of two (2) years post graduate clinical experience in a variety of treatment modalities.
