    Job Details

    Our Hospice Liaison RN’s :

    + Reviewing patient referrals, discuss hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care.

    + Develops a positive working relationship with doctors, nursing facilities, and hospitals who refer clients to hospice.

    + Coordinate discharges

    + Execute and follow up on referrals

    + Advocate for the patient and family and why they should choose Catholic Health

    + Act as a liaison to referral sources on behalf of Catholic Home Care and Good Shepherd

    + Identify potential opportunities for market growth and Hospice program expansion

    Qualifications:

    + Current New York State Registered Nurse (RN) license

    + Minimum of one year of recent hands on experience

    + Basic Life Support Certification

    + Current, valid NY State driver's license and own transportation.

     

    Salary Range

     

    USD $90,000.00 - USD $113,000.00 /Yr.

     

    This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
