Director of Population Health Details
Essential Job Duties:
+ Serves as the administrative liaison or leader for population health management operations for Union Health System.
+ Develops and drives the operational strategy at the system and local levels through effective communication to physicians and practices, implementing best practices, driving practice behavior based on actionable data, and working with key and various stakeholders at Union Health to ensure understanding of new or enhanced operations.
+ Develops and identifies strategies for high-risk patients through collaboration with physicians and practices, research, and population health focused analysis.
+ Capable of maintaining a focus on strategic organizational goals as well as perspective on operational details to promptly identify and resolve issues.
+ Leading the transition of innovative ideas from pilot to implementation based on best practices research.
+ Lead a high functioning team through shared vision, clear expectations, staff development, mentoring, and accountability.
+ Forward-thinking, well organized and self-directed with high ethical skills and an appropriate professional image.
+ Provides necessary tools and education to participate on/lead OP Quality, VBC, and Public Health or other project teams.
+ Supporting care delivery in a value-based care environment or model.
+ Driving operational action at the practice level targeting opportunities identified through monthly reporting, scorecards, and ad hoc analytics encompassing:
+ Risk Acuity (HCC Coding Accuracy)
+ Care Management
+ Quality
+ Appropriate Health Services Utilization
+ Cost
+ Preventative Care
+ Strong financial and analytical skills to develop and manage budgets, P&L, productivity, and key performance indicators.
+ Acts as liaison between physicians, APPS, practice staff and care management teams.
+ Attends professional meetings and conferences on current practices and trends in healthcare.
+ Performs other duties as assigned.
Qualifications:
+ Minimum of 3–5 years experience with management responsibilities.
+ Master’s Degree in Business Administration, Healthcare Administration, or Public Health required.
+ Minimum BS Degree from an accredited nursing program and current unrestricted Indiana Registered Nurse license.
+ A minimum of 5 years of current nursing is preferred.
+ Broad general knowledge of the structure, functions and activities of a Population Health program.
+ Must be accomplished in written and verbal business communication
+ Must be skilled in developing and presenting group presentations.
+ Experience in quality improvement activities and project management required.
+ Ability to flex schedule to accommodate staff needs.
+ Communication skills with diverse individuals.
