    About McIntosh Trail

     

    At McIntosh Trail Community Service Board, we offer individuals experiencing symptoms associated with mental illness, addictive disease and/or developmental disability the hope for optimal functioning and recovery by providing quality behavioral health services and supports.

    Summary

    As a case manager, you will be responsible for helping individuals with a substance use disorder or other co-occurring mental health disorder . Case managers provide advocacy for individuals and arrange transitional plans for clients to continue receiving care after discharge from the crisis stabilization facility.

    Responsibilities

    + Provide short term case management and referral services to individuals in a crisis.

    + Collaborates with the health care team and involves the individual/family in the development and implementation of the discharge plan.

    + Facilitates patient care across a multidisciplinary care continuum which includes interacting with Providers and other health care members to facilitate movement of the individual through the system and arrangement of an appropriate plan of care.

    + Work with individuals, families, support networks and health care professionals to put discharge plans in place.

    + Identifies issues that may delay individual’s discharge and facilitates a timely resolution of these issues and communicates anticipated/actual delays with the individual, family and clinical team.

    + Prepares clients discharge by reviewing and amplifying discharge plans, coordinating discharge and post-discharge requirements and providing resources.

    + Document individual’s progress including charting referrals and other notable interactions in a timely manner.

     

    Bachelor's degree and three (3) years case management experience; or one (1) year of experience at the lower level Child Welfare Svcs Case Mgr 2 (SSP071) or position equivalent.
