    Elevate Care is looking for a dedicated Infection Preventionist RN or LPN to lead our infection control efforts and support our mission of providing a safe, healthy environment for both residents and staff.     Key Responsibilities:

    + Develop, implement, and monitor the facility’s infection prevention and control program

    + Perform ongoing surveillance and investigate any outbreaks or infection trends

    + Provide training and education to staff on infection prevention best practices

    + Ensure compliance with CDC, CMS, OSHA, IDPH, and other applicable guidelines

    + Collaborate with clinical teams to reduce healthcare-associated infections (HAIs)

    + Conduct audits and maintain documentation for state and federal survey readiness

    Requirements:

    + Current Illinois RN or LPN license in good standing

    + Experience in infection prevention/control preferred

    + Certification in Infection Control (CIC) is a plus or willingness to obtain

    + Familiarity with long-term care or skilled nursing facilities is strongly preferred

    + Excellent organizational and communication skills
