Job Details:
The Case Management Assistant works closely with the case management team and completes tasks in discharge planning services for patients admitted to the hospital. They are a part of the multidisciplinary treatment team and must be able to function independently on a professional level.
Responsibilities:
+ Facilitate discharge planning referrals as guided by RN CM and SW across multiple units.
+ Provide patient/family with facility/agency lists for patient choice.
+ Prepare and send referrals for post-acute services through referral system to appropriate. facility/agency.
+ Complete and document in Allscripts all referrals made and include any updates or completions notes for all referrals.
+ Review worklist for new referrals throughout the day to ensure timely response.
+ Assist CM/SW with clerical support (faxing, follow-up calls) arranging for discharge of patients, initiating, and following up authorizations. Arrange transportation and/or DME for patients as needed.
+ Actively participate in multidisciplinary team rounds. Interacts with treatment team, patients, families, and community agencies in arranging individualized discharge plans.
+ Contribute in staff meetings, case conferences, case presentations and training activities. Maintain required documentation of work activities.
+ In collaboration with the case manager and care coordinator, implement the discharge review program mandated by New York State Department of Health to facilitate issuance of MOONs, IMM's, and discharge notices to patients.
+ Use computer applications such as spreadsheets, word processing, calendar, email, and database software in performing work assignments.
+ Access protected health information (PHI) in accordance with department assignments and guidelines defining levels of access.
+ Establish and maintain effective interpersonal relationships with patients and treatment team members.
+ Performs other duties as assigned.
Qualifications/Requirements:
Experience: 1 year of administrative and/or customer service experience, required Experience working in a healthcare setting, preferred.
Education: High School Diploma or equivalent, required Associate’s Degree, preferred
Licenses / Certifications: N/A
Other: Excellent written and communication skills. Commitment to enhancing cultural competency. Ability to prioritize and work in a fast-paced environment. Knowledge of Excel, Word, Outlook. Excellent oral and written skills, be organized and possess strong computer skills. Special Requirements: Knowledge of medical and pharmacology terminology, preferred. Knowledge of regulatory agency standards; Centers for Medicare & Medicaid Services, and Det Norske Veritas (DNV) and The Joint Commission standards, preferred.
