AdventHealth Tampa, a faith-based not-for-profit 626-bed tertiary-level acute care hospital located in the uptown district of Tampa, Florida, specializes in advanced cardiovascular medicine, neuroscience and neurology, digestive health, orthopedics, womens services, pediatrics, oncology, endocrinology, bariatrics, wound healing, sleep medicine and general surgery including minimally invasive and robot-assisted procedures. GME programs on the hospital campus currently include general surgery, obstetrics and gynecology, internal medicine and transitional year.

AdventHealth Tampa is home to the renowned AdventHealth Pepin Heart Institute, a recognized leader in cardiovascular disease prevention, diagnosis, treatment and leading-edge research caring for Tampa Bays hearts for over 35 years.

The Kiran C. Patel Research Institute, also located at AdventHealth Tampa, leads the way in clinical and academic research in the areas of neuroscience, cardiovascular medicine, advanced surgery and more contributing valuable knowledge that leads to tomorrows cures for conditions such as heart failure, cancer and other diseases.

The role you'll contribute:

The Executive Director of Care Management WFD will be responsible for developing and executing the strategic vision for care management across the WFD. This position will lead a care management strategy that enhances the consumer experience and effectively deploys Care Management resources. They will develop strategies supporting decreased variability in care management practices while working to embed evidence-based care transition practices. The executive director will partner with the SEO Clinical Outcomes, SEO Finance, and Corporate Exec Director of Care Management to develop strategies to reduce length of stay, reduce readmissions, improve STAR ratings for the division, and simplify daily workflows all while improving governance and accountability structures. The position will report directly to the SEO Clinical Outcomes and work collaboratively with the SEO Finance WFD, and Corporate Exec Director of CM, WFD Directors of CM, and members of the AdventHealth Clinical Team to align a vision for care management that supports the organizational strategic goals. Additionally, this role will have operational accountability for Post-Acute Networks, BPCI-A for WFD

Lead the execution of the strategic vision for care management across the WFD enhancing the consumer experience and effectively deploying Care Management resources **.**

Develop strategies to reduce length of stay, readmissions, while simplifying daily workflows, and supporting improved governance and accountability structures **.**

Implement policies to decrease variability in care management practices while working to embed evidence-based care transition practices.

Advise and support the standardization of care management roles, responsibilities, job descriptions and accountability metrics across the enterprise.

Coordinate the Director of Care Management WFD Council membership, agenda and deliverables.

Qualifications

The expertise and experiences you'll need to succeed** **:

Master's in business, healthcare administration, or Nursing Required

Bachelor's BSN Required

10 years of leadership experience in care management or related healthcare field Required

Case Management Certification Required
