Coordinator Pharmacy Services
+ The Medication Safety Coordinator serves as a key leader driving a culture of safety and continuous improvement in medication use across the north region. This role directs the review, triage, and resolution of reported medication safety events, ensuring timely follow-up by designated pharmacy stakeholders and maintaining comprehensive documentation of interventions and loop closures.
+ With a strategic, data-driven, and systems-oriented approach, the coordinator proactively identifies emerging safety trends and leads the development, implementation, and evaluation of process improvement initiatives to reduce risk and enhance medication safety. In partnership with the Manager of Quality and Medication Use Systems and the medication safety and quality pharmacist, the coordinator champions the local execution of system-level medication safety strategies, aligning them with regional goals.
+ Core responsibilities include leading pharmacy case reviews, Root Cause Analyses (RCAs), and Failure Mode and Effects Analyses (FMEAs) related to medication management. The coordinator serves as a primary consultant and decision-maker on safety issues, ensuring safeguards are embedded from design through implementation during workflow changes or policy updates.
Requirements:
+ Bachelor's Degree of Pharmacy from an accredited school of pharmacy.
+ State licensure as a Pharmacist.
+ 2 years post graduate experience.
Additionally, the Medication Safety Coordinator drives education and communication efforts, presenting safety trends and mitigation strategies to local and system committees, aligning nursing and pharmacy education initiatives, and mentoring pharmacy students and residents to cultivate strong medication safety practices within the profession.
