Wage: $30.90 per hour
Schedule: Monday-Friday 9AM-5PM
Why This Role is Perfect for You:
The Youth Clinical Case Manager for the DOHMH Court-Involved Youth Mental Health Initiative is responsible for assisting with clinical screenings, mental health assessment services, treatment planning and case management to court involved youth of the Youth Part in Kings County Supreme Court.
What You’ll Do:
+ Conduct biopsychosocial intake assessment with newly assigned participants as well as prepare and develop pre-plea investigations (PPI) to better inform sentencing.
+ Administer clinical screeners to secure age-appropriate referrals to counseling, education, programming, and vocational opportunities to ensure clients are fully supported.
+ Maintain court presence and produce formal written/verbal update to be shared on record for all court appearances.
+ Assist clients in obtaining benefits as indicated, including public assistance, medical and other needs.
+ Maintain complete client records, available for inspection by Supervision at any time.
+ Effectively schedule and maintain weekly client appointments to meet necessary requirements as well as monitor compliance (e.g., progress reports from educational programs, detention facilities, residential, and/or mandated treatment providers).
+ Maintain caseload of approximately 25 court-involved youth and/or their co-defendants (ages 14-24) through weekly outreach.
+ Schedule and attend on-site bi-weekly visitation with detained participants, as needed.
+ Assist with oversight of interns/volunteers and train as per agency and partner regulations if necessary.
+ Work with Supervisor to keep statistics on project goals including client contacts, referrals, job activities, etc.
+ Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.
What We’re Looking For:
+ Bachelor’s degree or equivalent required. Master’s degree preferred.
+ Minimum of one-year experience working with young adult population who experience substance use, mental health, complex trauma, and/or justice-system involvement.
+ Must have working knowledge of community treatment, support services and resources tailored towards youth.
+ Ability to work both independently and as part of a team.
+ Must be able to work independently and handle several complex projects at once.
+ Excellent communication skills, both oral and written, and the ability to work effectively with treatment and mental health systems, the criminal legal system, and/or alternative to detention/incarceration services.
+ Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments
