A Brief OverviewThis position supports patient centered care acting as an advocate to patients and their families by navigating them through the health care system and removing barriers to health. The Care Coordinator works specifically with high risk patients experiencing complex disease processes needing multiple sources of health care support. The Care Coordinator provides clinically based holistic care coordination in the inpatient and outpatient setting by working closely with clinic/hospital/specialty providers and staff, and community resources in a team approach to reduce patient risk of readmission.What you will do

+ Provides care coordination services for high risk and complex patient populations including but not limited to: ◦ Chronic Care Management ◦ ED and Inpatient utilization reduction ◦ Hospital discharge to home

+ Promotes behavior change and develops care plans for patients. Includes: ◦ The use of Motivational Interviewing concepts ◦ Increase comprehension through culturally and linguistically appropriate education and the use of evidence-based approaches such as teach-back methodology. ◦ Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s). ◦ Connect patients to relevant community resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs. ◦ Assessing and working on the patient’ readiness to change, the importance of change and confidence in ability to change. ◦ Assists all patients through the healthcare system by acting as a patient advocate and navigator. ◦ Supports patient self-management of disease and behavior modification interventions.

+ Participates in quality improvement initiatives.

+ Maintains accurate and timely documentation and patient tracking.

Qualifications:

Qualifications

+ Associate’s Degree (AA) Graduate of an accredited school of nursing (ADN) Required

+ Bachelor’s Degree in Arts/Sciences (BA/BS) In Nursing (BSN) Preferred

+ 1-3 years Hospital/clinic experience Preferred

+ Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.

+ Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

+ Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.

+ Knowledge of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects.

+ Understanding written sentences and paragraphs in work related documents.

+ Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.

+ Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.

+ Selecting and using training/instructional methods and procedures appropriate for the situation when learning or teaching new things.

+ The ability to combine pieces of information to form general rules or conclusions (includes finding a relationship among seemingly unrelated events).

+ The ability to listen to and understand information and ideas presented through spoken words and sentences.

+ The ability to communicate information and ideas in speaking so others will understand.

+ The ability to speak clearly so others can understand you.

+ The ability to identify and understand the speech of another person.

+ Basic Cardiac Life Support & First Aid - American Heart Association Preferred
