    JOB SUMMARY

    The Primary Care Prenatal Nurse is a registered nurse who specializes in providing high quality comprehensive care to individuals throughout the lifespan. This includes childbearing individuals, pregnant and postpartum persons, as well as providing support to their families. As part of a clinical team, the nurse works at the top of their licensure to enhance the safety, effectiveness, and efficiency of care through both the performance of professional nursing responsibilities and the delegation of technical and clerical functions as appropriate to medical assistants. The Prenatal Nurse is key to providing support for our providers, safe care for our patients, and ultimately improving clinical patient outcomes. The Prenatal Nurse primarily assists in the care coordination our perinatal patient's need, while serving as a teacher, advocate, and navigator for these patients within the context of the care team. Assumes responsibility, in collaboration with providers to coordinate care, support transitions between levels of care, and counsel, educate, and support patients in improving health status. Applies principles of advanced access, planned care and the chronic care model, and rapid cycle performance improvement using the PDSA approach at CHCI. The knowledge, skills, and expertise a good prenatal nurse brings to the job can be invaluable to expectant mothers and their babies.

    ROLE AND RESPONSIBILITIES

    + Support primary care of patients under planned care model, including pre-visit planning, daily huddles, and execution of nurse-led and nurse-assisted visits.

    + Triage of patients (both in person, and over the phone) based on clear triage protocols.

    + Collaborates with PCP to identify needs for preventive care, evidence based chronic disease care, and care coordination.

    + Provides general and specific health care education during patient visits and by telephone.

    + Works with patients to identify and monitor self-management goals.

    + Collaborates and coordinates care with other community based providers. Including attending and presenting at Continuation of Care Collaboration meetings with the local hospital child birthing center.

    + Follows up on emergency room visits and hospital discharges. Ensuring OB discharge follow up has occurred.

    + Assists providers with exams, treatments, and procedures as needed.

    + Performs placement and monitoring of patients for prenatal NSTs.

    + Ensures safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.

    + Administers medications, including immunizations, at the order of prescribing provider and under standing orders as established by CHCI.

    + Adheres to CHCI protocols regarding documentation of medication, route, dosage, and information provided to patient.

    + Monitors patient response to medication based on specific medication administered; detects, reports, and responds to any adverse reaction. Reconciles medications. Provides patient education as appropriate to the administered medication.

    + Performs wide range of nursing activities as needed and after initial prescription by provider such as wound care, HTN follow up, Rhogam administration, gestational diabetes education, etc.

    + Performs a wide range of nursing visits under CHCIs standing orders or delegated visits by the provider. Examples of standing order nursing visits include (but not limited to): Prenatal intakes, uncomplicated UTIs, titration of long acting insulin, treatment of vulvovaginal candidiasis, and pregnancy testing.

    + Accurately documents all aspects of patient care in the medical chart on a timely basis.

    + Maintains strict infection control standards in accord with CHCI protocols and universal precautions JC, OSHA and CLIA standards.

    + Acts as a resource person for medical assistants regarding patient preparation, needs and procedures needed.

    + Monitors and ensures follow up of all abnormal cancer screening results (PAPs and Mammograms), STIs, and other tests and screens performed at CHCI.

    + Oversees the maintenance of supplies and equipment, ordering supplies as necessary.

    + Maintains compliance with agency mandatory annual training (Patient Safety, Abuse, Age Based Competencies, EOC, Abuse trainings, etc.)

    + Expert use of electronic health record to ensure timely, safe, confidential storage and retrieval of patient information.

    + Participates in Project ECHO complex care management to continually advance knowledge and skills in care management and care coordination.

    + Presents nursing information and insight into patient care with other members of the care team at Integrated Care Team meetings.

    QUALIFICATIONS

    + Preferred Skills

    + Bachelor’s degree in Nursing (BSN)

    + Experience in prenatal or maternal health care setting

    + Knowledge of current prenatal care guidelines and practices

    + Strong understanding of prenatal and maternal health

    + Ability to work in a highly collaborative team environment

    + Excellent observational and diagnostic skills

    + Proficiency in electronic medical record (EMR) systems

    + Knowledge of ASCCP guidelines

    + Bilingual abilities are a plus

    + Licenses/Certifications

    + Current CT Nursing License

    + American Red Cross or American Heart Association (AHA) CPR certification

    + Certification in Obstetric Nursing or Maternal Newborn Nursing preferred

    + Electronic Fetal Monitoring certification (C-EFM) preferred

    PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

    + Frequent and alternating standing, walking, and sitting.

    + Occasional bending, squatting, kneeling, lifting.

    + Daily and continuous computer use.

    + Daily and frequent telephone use.

    ADDITIONAL ACTIVITIES/RESPONSIBILITIES

    + Committee membership.

    + Participation in quality improvement projects.

    + Precepting of nursing students and new CHCI nurses.

    + Participation in on going nursing education activities.
