Position Summary:
The Licensed Vocational/Practical Nurse, under the directionof a Registered Nurse, is responsible for providing nursing care, healthteaching, and health counseling to assigned patients during a given shift. TheLVN/LPN assures the delivery of care by assisting in the implementation of theinterdisciplinary and nursing plan of care.
Essential Duties and Responsibilities:
As an LPN in our inpatient rehabilitation setting, you willplay a vital role in supporting the nursing team and ensuring patients receivehigh-quality care during their recovery journey. Your responsibilities willinclude:
Patient Care: Assisting Registered Nurses with patient assessments,vital signs monitoring, medication administration, wound care (per RNsupervision), and assisting with activities of daily living (ADLs).
Rehabilitation Support: Working closely with therapists and otherhealthcare professionals to provide comprehensive rehabilitative services.
Patient Education: Educating patients and families about their carepractices and promoting a positive, encouraging environment.
Documentation & Communication: Documenting patient activities,observations, and responses accurately, and communicating them under thedirection of the RN.
Team Collaboration & Safety: Collaborating with theinterdisciplinary team to maintain a clean, organized patient care area andensuring patient safety protocols are followed.
Minimum Qualifications (Knowledge, Skills, Abilities):Licenses or Certification:
- Current unencumbered license to practice as a vocationalnurse in the state where currently practicing.
- Current BLS via American Heart Association or American Red Cross required.
