    Job Summary and Qualifications

     

    As a **Trauma Complex Care Social Worker Case Manager,** you will assist our inpatient trauma patients in finding the services they need for continuity of care upon leaving the hospital. Our team collaborates closely with our community partners to provide services such as home health, hospice, skilled nursing, assisted living, private care, and durable medical equipment (DME).

     

    Inpatient trauma care relies on a multitude of interrelated surgical and medical specialties, diagnostic tests, therapeutic interventions, and allied health services to reduce mortality and morbidity from severe injuries. The complexity of this care can often overwhelm the ability of the primary medical team to ensure that the care is timely, well organized, and efficient. As a **Trauma Complex Care Social Worker Case Manager** , you will assist in ensuring that the myriad of details of care are neither forgotten (potentially leading to higher complication and mortality rates) nor duplicated.

    What you will do in this role:

    + Perform thorough assessments and develop appropriate plans of care for high risk patients with discharge planning or psychosocial needs.

    + Review of patient notes identifying and addressing any conflict in medical orders or lack of management plan.

    + Build a rapport by providing continuity of care with patients and acting as their advocate throughout their hospital stay.

    + Collaborate between multiple care givers and foster communication between medical teams and nursing staff

    + Participate in interdisciplinary care planning process with the Trauma Team.

    + Identify and intervene to coordinate care of trauma patients in relation to appropriateness of level of care, length of stay and resource utilization issues.

    + Identify barriers to discharge and contacting relevant personnel to overcome these.

    + Initiate and monitor referral process of patients post discharge.

    + Identify, track, and record Avoidable Days [AD], intervening to prevent or decrease as appropriate.

    + Initiate denial and appeal process in accordance with policy.

    + Demonstrate awareness of current trends that influence health care practice and assures compliance with professional, regulatory, governmental and organization standards.

    What qualifications you will need:

    + Graduate of an accredited school of social work.

    + Master's degree in Social Work (MSW).

    + Licensed Clinical Social Worker preferred.

    + 3 or more years of social work experience in an acute care setting preferred.

    + Previous case management or utilization review experience preferred.
