    Job Description

    The Advanced Practitioner /Clinical Program Director for Wound and Ostomy care is responsible for the development of a comprehensive Wound, Skin and Ostomy care program and insures the delivery of prevention and care in accordance with the philosophy, standards of practice, objectives and policies of the Department of Patient Care Services. The Director provides advanced clinical leadership, strategic oversight and operational direction for the program within the surgical clinical practice. The Director is responsible for diagnosis and treatment of common problems related to the treatment, management and rehabilitation of patients with ostomies, wounds and/or continence problems as well as patient and family education and counseling and education of nursing staff. S/he is accountable for the budget planning and resources to enhance the quality of patient care, personnel and resource management, and in collaboration with senior nursing leadership, the annual goals, objectives and fiscal targets. The Director serves as an expert clinician, mentor and administrator, enhancing the patient and family experience, improving clinical outcomes and fostering interdisciplinary collaboration.

    Qualifications

    Education requirements

    Minimum of a Master’s Degree in Nursing preferred

     

    Graduate of an accredited Nurse Practitioner program

     

    Graduate of accredited Wound, Ostomy, and Continence Educational Program

     

    Bi-lingual preferred

    At least two years of nursing experience

     

    Nurse Practitioner experience of a least one year preferred

    Responsibilities

    Patient Experience

    1. Serve as the lead advanced practice provider for wound and ostomy management in the surgical clinical practice.

    2. Deliver expert consultation and direct patient care including assessment diagnosis and management of complex wounds ostomy and continence needs.

    3. Develop and implement individualized treatment plans that reflect patient and family goals

    4. Assure continuity of care and optimal outcomes for patients and families.

    5. Follows up on all patient concerns in a timely manner, provides service recovery as needed and escalates patient concerns as appropriate.

    6. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions.

    7. Addresses issues of communication, emotional support, and the complexities of managing wound, skin and ostomy conditions to prevent patients feeling overwhelmed by their conditions, and ineffective care can lead to feelings of neglect or frustration.

    8. Promote an excellent patient experience by ensuring compassionate communication, timely interventions, and shared decision making.

    9. Advocate for patient and family engagement in self-care management, emphasizing dignity autonomy and cultural sensitivity.

    Essential Duties and Responsibilities

    1. Develop, revise and evaluate standards of care

    2. Assist in the selection of supplies, technology and equipment

    3. Assure appropriate product utility while maintaining optimal patient safety and outcomes; identifies rental situations when appropriate

    4. Develop, provide and evaluate staff education programs, ie. Orientation, in services and continuing education

    5. Conducts health assessment interview focused on wound, ostomy, continence related care

    6. Performs focused physical assessment as appropriate.

    7. Orders and interprets diagnostic tests.

    8. Develops plan of care based on the diagnosis and individual patient needs.

    9. Prescribes medications and treatments as appropriate.

    10. Works collaboratively with the health care team and makes referrals to other disciplines as indicated.

    11. Records, reports and interprets the client’s responses to the plan of care.

    12. Teaches, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems

    13. Provides individualized fitting for appropriate medical equipment for in-hospital and outpatient ostomy patients

    14. Maintain a minimum of 4-6 hours per week (scaling up to 8 hours based on patient demand) in direct, outpatient care within the surgical clinical practice, including but not limited to outpatient initial and follow up visits.

    15. Influence reductions in readmissions and length of stay through proactive outpatient management.

    16. Participate in surgical case planning and postoperative follow up for ostomy patients

    17. Ensure availability for advanced consults and staff support for both inpatients and out-patients on complex wound and ostomy cases.

    Quality and Safety

    1. Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice.

    2. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and develops and implements corrective action plans.

    3. Fosters a fair and open culture that encourages error and near-miss occurrence reporting.

    4. Leads interdisciplinary quality improvement teams using the principles of high reliability.

    5. Communicates with staff and leadership about unit progress, changes, and compliance with quality and safety metrics.

    6. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol.

    7. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation.

    8. Fosters a professional practice environment through facilitation of collegial interdisciplinary relationships, peer review, nursing research, innovation and joint practice arrangement through affiliating universities.

    9. Lead quality improvement initiatives focused on reducing hospital acquired pressure injuries and ostomy related complications

    10. Monitor and report quality metrics related to wound and ostomy outcomes; Design action plans to address gaps.

    11. Collaborate with infection prevention surgical services, and nursing leadership to promote evidence-based protocols.

    12. Champion a culture of safety through staff education audits and feedback systems

    Research and Innovation

    1. Integrate research findings into practice; support clinical trials and product evaluations in wound and ostomy care.

    2. Lead or participate in scholarly projects, publications, and professional organizations.

    3. Present and publish evidence-based practice initiative at regional and national conferences

    4. Contributes to the profession of nursing through leadership and participation in professional organizations.

    5. Evaluate emerging technologies, products and therapies to improve care delivery and outcomes.

    Operations and Administration

    1. Supports the implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units.

    2. Works collaboratively with other Directors/Nurse Managers/Assistant Nurse Managers, physician dyads and all members of the healthcare team to develop a cohesive wound and ostomy program.

    3. Maintains service line compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission.

    4. Oversees Nurse Managers/Assistant Nurse Managers to ensure 24 hour patient care delivery, throughput, staffing and processes to insure efficient and effective operations.

    5. Maintains patient/employee confidentiality in the management of information.

    6. Monitors availability and maintenance of nursing equipment and supplies.

    7. Encourages, recognizes and rewards manager and staff participation in quality and safety initiatives, compliance and successes.

    8. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives.

    9. Provide strategic direction and oversight for the wound and ostomy program including resource allocation and budget management.

    10. Develop, update, and enforce policies, procedures, and clinical guidelines related to wound and ostomy care.

    11. Supervise, mentor, and evaluate WOC nurses and support staff, fostering professional accountability and excellence.

    12. Partner with surgical and nursing leadership to align program operations with institutional goals and patient needs.

    14. Coordinate interdisciplinary collaboration with physicians, dieticians, physical therapists, and case management.

    7. Acquires and disseminates knowledge and skills relative to wound and ostomy care, prevention and treatment.

    8. Serves as a professional role model and preceptor for new leadership, staff and students that supports ongoing clinical education and skill development.

    9. Establishes and implements a system that supports best practice patient care decisions, safe and efficient patient throughput and access to resources for the unit/clinical service area.

    10. Design and lead training programs for nurses, providers, and ancillary staff on wound and ostomy best practices.

    11. Provide structured patient and family education on wound care techniques, ostomy management, and lifestyle adaptation.

    12. Support continuing education, certification and advancements for staff within the WOC specialty

    13. Incorporates the values of research and evidenced based practice and its application to the environment of care.

    Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $139748 - $209622 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
