    Responsibilities*

    Serve as a clinical resource and consultant in a multi-disciplinary setting to patients, families, nurses, and medical staff specializing in colorectal surgery

     

    Perform daily phone and triage tasks in MiChart related to patient concerns, issues, and needs.

     

    Care is focused on young adults, adult, and geriatric patient population. Pediatric care will be provided as deemed appropriate.

     

    Provide coordination of nursing care, education, and support to patients and families of the Multidisciplinary Colorectal Cancer and Benign Breast Clinics, as well as other surgical patients.

     

    Coordinate care pre-operatively and post-operatively for the surgical patient population primarily in the Colorectal Surgery and Benign Breast outpatient setting. This will include allocated time in the Taubman and/or Rogel Cancer Center clinic setting.

     

    Assist in the development of patient care standards in collaboration with the clinical management team.

     

    Assist in the development and implementation of competencies, protocols, and patient education tools to improve patient care.

     

    Participate in the development of institutional policies, procedures, and guidelines in keeping with evidenced-based practice and standards for nursing care of patients with colorectal disorders.

     

    Work collaboratively with all members of the general surgery team to provide efficient patient care processes, especially for the Benign Breast and colorectal surgery patient population.

     

    Assist in the administration and coordination of the multidisciplinary care of rectal cancer patients in support of the Rogel Cancer Centers participation in the National Accreditation Program for Rectal Cancer.

     

    Order tests, renew prescriptions, inform patients of results from testing, telephone triage (gathering information, giving instructions and answers to patients, teaching, relaying problems to MDs, and making decisions for patients to come to ER or clinic using nursing assessment skills).

     

    Participate in quality assurance/quality improvement, evidenced-based practice, and/or LEAN initiatives.

     

    Support research initiatives.

     

    Participate in department and/or division committee meetings to improve patient care across the care continuum.

     

    Serve as clinical care coordinator for the Benign Breast practice and the multidisciplinary colorectal cancer program by attending and assisting with administration of tumor board, registering, monitoring, and proactively navigating patients with suspected and confirmed colon and rectal cancer throughout their diagnostic, treatment, and surveillance pathways in a timely manner.

     

    Oversee that rectal cancer patient care pathways are followed according to agreed guidelines and timelines for relevant interventions. Rectal cancer care coordination includes, but is not limited to, communicating between departments with the facility, referring physicians, and patients.

     

    Liaise with relevant departments to assemble all pertinent information for the rectal cancer tumor board meetings and communicate and enact tumor board findings/recommendations with referring organizations and/or providers.

    Salary & Nursing Framework Level:

    This UMPNC RN posting is posted under the Role-Specific Advancement Model (RSAM) as:

    CCC- COMPETENT NE.

     

    Actual RSAM LEVEL and salary will be determined at time of hire.

     

    RSAM levels range from CCC-COMPETENT NE to CCC-EXPERT NE TO CCC-MASTERY NE or CCC-Mastery Plus NE.

     

    Required qualifications must be met by the candidate in order to be interviewed and considered for the position. Posting may be filled after the initial 5-day posting period.

     

    Applicants who have left the UMPNC bargaining unit must include on their resume dates of past employment including months and years of service along with effort.

     

    Positions less than 20 hours/week may be combined.

    Required Qualifications*

    A current license in the State of Michigan as a Registered Nurse.

     

    Educational Requirement; applicant must meet one of the following: Bachelors degree in Nursing OR an Associates degree or diploma in Nursing **and** a Masters degree in Nursing

     

    Five (5) years of RN experience

     

    Three (3) years of RN experience (within the last 5 years) of direct inpatient or outpatient care for an adult surgical patient population

     

    In order to be considered for this position the applicant must have met or will have met all the required qualifications prior to the start date of employment.

    Desired Qualifications*

    Two (2) or more years of experience with the oncology patient population

     

    Two (2) or more years of experience with the gastrointestinal surgery patient population

     

    Experience managing surgical patients in the outpatient setting

     

    Ambulatory care phone triage

     

    Experience providing patient education

     

    Two (2) or more years of recent (in last 3 years) experience with electronic medical records, specifically EPIC

     

    Computer/typing skills such as Microsoft Word and Excel

    Work Schedule

    ? Hours per week: 40 hours

    ? Shift: Days; Monday-Friday, variable times. Weekends and Holidays as patient care demands

     

    All employees will be expected to float to a designated unit(s) in times of low census or department needs.
