    Responsibilities:

    Provide intensive support and follow-up services to consumers to successfully integrate them into their communities. As a Day Hab Counselor, you will transport and accompany seniors / other age groups in activities such as bowling, movies, volunteer work, socialization and other recreational activities. Additional duties, such as assisting with personal care needs, may be required.

    Qualifications and Education:

    A minimum of a high school diploma or equivalent and prior experience working with adults with disabilities required. Bachelor’s degree preferred. Valid NYS Driver’s License required.
