Opportunity Details:

+ Shared single entrance and waiting area with Family Medicine with dedicated Internal Medicine exam room and physician workspace

+ 36 patient contact hours/ week

+ Patient volume: 18-20

+ 100% Outpatient

+ Outpatient Internal Medicine call 1:18 (Second Call)

+ Chronic care coordination team, behavioral health support services

+ On-site X-ray and lab services
