    Per Diem Social Worker needed for Boston Children’s Hospital, Longwood Primary Care Clinics

    Key Responsibilities:

    + Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.

    + Performing psychosocial and risk assessments, assigning diagnoses, and developing and implementing interventions and treatment plans.

    + Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan

    + Implementing and following up on treatment and provision of needed services.

    Minimum Qualifications

    Education:

    + Master’s degree in social work

    Experience:

    + Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.

    + Experience with social services agencies, understanding of social determinants of health and trauma ideal.

    + Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!

    Licensure/Certifications:

    + Active LICSW in the state of Massachusetts
