Licensed Practical Nurse (LPN) – Community-Focused Primary Care
Job Type: Contract-to-Hire
Schedule: Monday–Friday, with 1–2 evenings per week (late start on those days)

What You’ll Do
+ Monitor and document vital signs, patient measurements, and clinical observations.
+ Prepare exam rooms and manage medical supplies to ensure a safe, organized, and hygienic environment.
+ Assist with patient intake, follow-up care, and education.
+ Maintain accurate and timely documentation in Electronic Medical Records (EMR) systems.
+ Collaborate with clinicians to anticipate patient needs and support care delivery.
+ Serve as a resource for nursing-related questions and potentially mentor other LPNs.
+ Coordinate patient scheduling and medical testing logistics.
What You Bring
+ Licensure: Active LPN license in the state of New York.
+ Experience: 1–2 years in a medical office setting; 2+ years preferred.
+ Skills: Strong clinical judgment, patient care abilities, and reliability.
+ Technology: Experience with EMR systems; EPIC preferred.
+ Certifications: Basic Life Support (BLS) certification is a plus.

Pay and Benefits
The pay range for this position is $28.00 - $35.00/hr.
