Essential functions (job duties):
Job Purpose:
Provide case management services to HIV positive teens and young adults within the Horizon Project. Participate in the creation of care plans. Guide and monitor clients through the full patient care cycle from care planning to case closure. Assist clients in accessing social work, counseling and other community-based resources. Work as a team member with other staff, ensuring a holistic approach to treatment/services is being followed for each client. Connect with community agencies servicing the HIV/AIDS population to make transitions and access easier for clients.
Essential Functions:
Provide case management service and assistance with navigating the health system to individuals infected and affected by HIV/AIDS. Increase identification of HIV positive teens and young adults through testing. Decrease barriers to access of care by facilitating psychosocial needs assessments to identify potential barriers to care. Work with clients to provide resources that are needed to eliminate/reduce barriers to treatment that may include, but not limited to, transportation, housing, insurance, and education.
Provide care coordination, data management and liaison with other HIV/AIDS organizations providing service to target population. Develop and maintain positive working relationships and partnerships with other social/human service providers as it relates to the Horizons Project to increase testing and referral efforts for the targeted population.
Conduct intake and follow-up for HIV infected and affected persons requesting services from the Horizons Project which includes medical and mental health care. Develop individual service plans and adjust plans as needed. Monitor status and needs of clients through regular contact with clients to assure changes are addressed as per standards of care.
Assess mental status and monitoring of functioning. Provide ongoing supportive counseling to address mental health needs and substance use disorders including adjustment, identity, depression, family, and relationship. Assist clients with strategies to head off crisis and encourage stability. When necessary, communicate with friends, families and/or healthcare professionals to facilitate the continuum of care or provide additional support/resources.
Provide hands-on support in accessing resources and services including completing paperwork to obtain insurance, housing assistance, dental appointments or Supplemental Security Income. Accompany youth to Department of Health and Human Services for medical appointments and/or procedures or to obtain other services; Advocate for services on behalf of clients for identified needs and eligibility of services including food, housing, medication, educational, vocational.
Participate in collection and reporting of data documentation of internal and external reports as mandated by funding and the Horizons Project policies. Maintain strict confidentiality of all client related data in accordance with state/local confidentiality laws. Attend Multidisciplinary Team, Staff and grant related meetings.
Perform other related duties as assigned.

Qualifications:
Education: Bachelor's degree
Bachelor’s degree in Health and Social Sciences such as Social Work, Psychology, or nursing. Candidates with equivalent combination of education and experience may be considered.
Experience:Experienced (minimum 2 years of job-related experience)
Experience in working with adolescents and/or HIV/AIDS population preferred. Training in counseling and substance use disorders, required. Must complete state-sponsored HIV prevention counseling and testing training/certification within 3 months of hire. This position is a combination of a social worker, clinic-based case manager, and community-based case manager.
