Role Overview:

As an Intake Specialist, you will receive, interpret, and manage patient referrals to help convert them into admissions. You will maintain organized systems and communication channels to ensure continuity of patient care and respond to the needs of both internal and external customers. This role supports Elara Caring’s mission by managing referrals efficiently and contributing to patient care coordination.

Key Responsibilities:

+ Receive and manage patient referrals with the goal of converting to admissions

+ Work pending lists to improve referral conversion rates

+ Respond to and satisfy expectations of internal and external customers

+ Maintain organized and efficient systems to ensure continuity of care

+ Enter referral information accurately into company database software

+ Attend meetings with branches and referral sources as requested

Qualifications:

+ High School Diploma or GED required

+ Minimum 1 year of healthcare experience with knowledge of medical terminology

+ Proficient with Microsoft Office Suite and data entry

+ Strong customer service skills

+ Associate degree in healthcare preferred
