Join My Care of Ohio’s Case Management Team and use your clinical expertise to improve lives across Clinton, Clermont, and Hamilton Counties.
Position Details
**Title:** Case Manager, Licensed Social Worker (LSW)
**Location:** Must reside in Clinton, Clermont, or Hamilton County, OH
**Schedule:** Monday–Friday, 8:00 AM – 5:00 PM EST
**Work Type:** **Hybrid** community-based care role
**Travel:** 50–75% local travel to member homes or nursing facilities (mileage reimbursed)
**Pay Range:** $35.95 – $37.99/hour (based on experience)
Responsibilities
+ Conduct comprehensive assessments to determine member needs and eligibility
+ Develop individualized care plans to enhance wellness and independence
+ Facilitate member access to community and healthcare resources
+ Use clinical judgment to navigate complex health and social issues
+ Collaborate with multidisciplinary teams to ensure optimal outcomes
+ Maintain documentation in line with regulatory and company standards
Qualifications
+ Active LSW license in good standing (State of Ohio)
+ 3+ years of behavioral health or social work experience
+ Strong communication, organization, and problem-solving skills
+ Reliable transportation and willingness to travel locally
+ Preferred: Case management experience, community-based social work, and proficiency in Microsoft Office

**Pay Details:** $35.95 to $37.99 per hour
