Job Description
We are looking for a highly qualified and career-minded **Licensed Practical Nurse (LPN)** to work as a **LPN Clinical Intake Coordinator** in our 57 bed skilled nursing center.
Position Details :
Licensed Practical Nurse (LPN) – Clinical Intake Coordinator
Schedule: Monday - Friday; Days; 8a-5p
On-call weekends for admissions
Supportive environment where residents and staff are valued
The Clinical Intake Coordinator facilitates the home health referral process by receiving referrals from multiple referral sources within and without the St. Luke's Health System, performs initial evaluation of referral for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals. Facilitates and promotes the Tele-Health services for the agency. This position may require travel.
