    Responsibilities:

    As part of the Care/Case Management department, the Senior Community Health Navigator supports the development of and execution of Blue Cross Complete of Michigan’s member outreach strategy to impact health outcomes, health care costs, and member engagement. This position establishes and cultivates relationships with members, community stakeholders, and providers via phone, mail, email, and personal visits. Face-to-face member visits at their home or community organization are an essential expectation of the Senior Community Health Navigator role.

     

    The Senior Community Health Navigator may be asked to focus on one or all of the following functional areas. ; ;

    Member Outreach & Community-Based Partnerships:

    + Maintain relationships with contracted entities providing community health worker services.

    + Facilitate data exchange to and from contracted entities.

    + Develop a strategy for navigator community partner visits to engage members to improve member health outcomes.

    + Assists in the maintenance of community health resources on the findhelp tool. ;

    + Supports outreach for the Community Care Management Teams (CCMT) and additional HEDIS measures. ;

    + Develops and maintains relationships with new and established community-based organizations to promote strong partnerships.

    + Provide health education, instruction, and information so that clients can easily understand and address barriers to physical and mental health services.

     

    In addition, the Senior Community Health Navigator is responsible for taking on other responsibilities that support the day-to-day operations. ;

     

    + Contributes to developing and advancing community health strategies and activities.

    + Partners with leadership to identify navigator strategies that need strengthening or discarding and implement additional strategies needed to continue advancing the community health navigator program. ;

    + Facilitates training and information to new, contracted, and existing associates. ;

    + Mentor, coach, and guide Community Health Navigators, as assigned by their supervisor.

    + May assist with special projects and other duties as assigned by their supervisor.

    + Assists in creating a positive and supportive work environment; enforces a safe workplace; establishes a culture of teamwork and communication; creates a workplace that promotes the organizational values of workplace diversity, equity, and inclusion.

    + May pick up and deliver supplies and equipment, including pick up/transport of materials from provider offices.;

    Education/ Experience:

    + Bachelor’s degree in a health education, healthcare administration, human services, or related field, or equivalent work experience.

    + Minimum of three years in the CHN role.

    Other Skills:

    + Required: Community-based experience through volunteerism, employment, or education.

    + Knowledge and understanding of managed care, medical terminology, and disease processes.

    + Ability to effectively communicate preventive health and health management philosophies to members, small groups, and external agencies.

    + Comprehensive knowledge of Medicaid requirements, plan policies, and procedures.

    + Must have a valid Michigan Driver’s License, valid car insurance, and reliable transportation.

    + Traveling to and accessing members' homes and community locations within the assigned territory.

    + Must be able to travel to Wayne, Macomb, and Oakland counties.

    + Must be able to lift to 25 pounds.

    + Bilingual preferred.;
