    Amergis is hiring a compassionate and experienced NP or PA to join an PACE Center, serving the elderly population of Monmouth County. This position offers the opportunity to make a meaningful impact in a collaborative care environment.

     
     

    + Provide direct and indirect care to participants aged 55+

    + Manage complex clinical situations under Medical Director supervision

    + Conduct assessments, follow-ups, sick visits, and occasional home visits

    + Maintain a patient panel of ~100 participants

    + Use PCO (PACE Care Online) EMR system

    ? Qualifications:

    + Certifications:

    + PA: Board Certified through NCCPA

    + DEA & BLS required

    + Licensure:

    + Active NJ license

    + Experience:

    + 1–2 years of recent primary care experience with elderly patients (ages 55+), treating both male and female patient

     
     

    + CMA and nursing support provided

     
     

    + 30-minute virtual interview via Microsoft Teams with leadership
