Maxim Healthcare is seeking a dedicated and compassionate Coordinated Family Support (CFS) Specialist to help adult clients with developmental disabilities thrive while continuing to live in their family homes. This role focuses on building life skills, coordinating essential services, and fostering community integration—both at home and in the community.

What You’ll Do:
+ Coordinate and facilitate services that support clients’ ability to live independently in their family home.
+ Assist clients and families in navigating and accessing community resources and “generic services.”
+ Provide support and training in daily living skills, both in-home and in the community.
+ Communicate regularly with families to ensure alignment and progress.
+ Help schedule and coordinate medical and other essential appointments.
+ Supervise clients during community outings as needed.
+ Maintain accurate and timely documentation, including quarterly progress reports.
+ Uphold strict confidentiality and report changes in client status per care plan guidelines.
What You’ll Need:
+ State licensure or certification as required by state/program.
+ Associate degree in the human services field or 3+ years of experience in developmental disabilities services.
+ Experience writing progress reports on a monthly and quarterly basis
+ Experience with submission of notes to report on weekly progress
+ Current BLS or CPR/First Aid certification.
+ Current PPD or Chest X-ray clearance.
+ Strong coordination and communication skills to work across home, school, and community settings.
+ Proficiency in English and basic computer skills.
+ Must be at least 18 years of age.
