    Description

    Step into a vital role at New Century Health Plan and coordinate the care for our Medicare Advantage members. As a Clinical Social Worker, you will focus on addressing members' complex social needs to help improve health outcomes and streamline their care journey.

     

    In this position, you will work closely with members, their families, community organizations and healthcare providers. Your responsibilities will include:

     

    + Providing a range of social work services responsive to identified problems (e.g., psychosocial functioning, adaptation to illness, facilitating services, protective services, post-hospital planning, etc.)

    + Maintaining assigned care management caseload with a focus on the most complex, highest-risk members particularly those with advanced chronic conditions, co-occurring mental and/or substance abuse and complex social issues (e.g. homelessness, domestic violence).

    + Assessing members' healthcare needs through comprehensive psychosocial assessments.

    + Collaborating with Nurse Care Managers to provide support to members enrolled in the Complex Care Management and Disease Management programs.

    + Providing education and resources to members about accessing services, community resources, and self-management skills and strategies.

     

    Salary Range: $54.54-$65.18/hourlyQualifications

     

    We're seeking a detail-oriented, intrapersonal, licensed professional

    with:

    Required:

    + Master's Degree in Social Work, or equivalent from a school accredited by the Council on Social Work Education

    + Current unrestricted license (LCSW) with the California Board of Behavioral Sciences

    + Three or more years of Case Management/Care Coordination experience in a healthcare setting

    + Two or more years of experience in management of individuals with complex acute or chronic conditions and psychosocial needs

    + Ability to triage member health and safety risks and prioritize interventions

    + Proficiency in electronic health records (EHR) and care management software

    + Ability to assess and analyze complex cases to develop individualized care plans and interventions

    + Knowledgeable of community resources and services

    + Exceptional interpersonal, written, and verbal communication skills

    Preferred:

    + Certified Case Manager (CCM), preferred

    + At least one year of experience in a managed care or health plan setting, preferred

    + Minimum of one year of experience with Medicare Advantage plans and knowledge of CM regulations and NCQA requirements, highly desired
