Position Purpose:
The School Health Services Liaison is a vital member of SilverSummit HealthPlan and is responsible for building strong partnerships with schools and school districts to promote access to care coordinating and encouraging uptake of Medicaid and Nevada Check Up covered School Health Services. This position serves as the primary point of contact between the health plan and educational institutions, working to improve health outcomes, reduce disparities, and support the holistic well-being of school-aged members.
+ Cultivate trusted relationships with school nurses, administrators, parent liaisons, and district wellness coordinators.
+ Perform outreach activities with each school district in Nevada at least quarterly.
+ Provide education to school personnel and families about Medicaid-covered services, EPSDT benefits, behavioral health, and member support programs.
+ Partner with school staff to identify students with unmet physical, behavioral, or dental health needs.
+ Facilitate referrals to in-network providers, including mobile clinics, telehealth, and school-based care options.
+ Assist families in navigating care access barriers such as transportation, language needs, or complex care coordination.
+ Coordinate on-site wellness events, vaccination drives, behavioral health education sessions, and preventive screening programs.
+ Maintain logs of school outreach activities, member engagement efforts, and follow-up outcomes.
+ Provide regular reports to internal leadership on utilization trends, outreach barriers, and opportunities for improvement.
+ Performs other duties as assigned.
+ Complies with all policies and standards.
Education/Experience:
Bachelor's Degree in Public Health, Social Work, Education, Nursing, or a related field required.
+ 2+ years experience in community health, Medicaid programs, school-based services, or member engagement required.
+ Demonstrated understanding of Medicaid/CHIP benefits, EPSDT, and social determinants of health required.
+ Experience working directly with schools or educational agencies preferred.
+ Knowledge of local community resources and health care delivery systems preferred.
+ Bilingual in Spanish or other locally spoken languages preferred.
+ Frequent travel to school sites, district offices, and community-based events. Occasional evening or weekend availability may be required for school-related activities required.
Licenses/Certifications:
LVN - Licensed Vocational Nurse Must live in the state of Nevada
LCSW- License Clinical Social Worker
Pay Range: $55,100.00 - $99,000.00 per year

Actual pay will be adjusted based on an individual's skills, experience, education, and other job-related factors permitted by law, including full-time or part-time status. Total compensation may also include additional forms of incentives. Benefits may be subject to program eligibility.
