Our mission is to offer people with disabilities the necessary supports in order to promote the personal values of individual growth, participation, contribution and capability.
Organizational Focus:
We provide services to individuals with developmental/intellectual disabilities living in Colorado. Admissions are for individuals with complex behavior and medical needs who may pose a risk to themselves or the community. We provide emergency treatment stabilization for people and provide support to the community service delivery system.
Overall function of the work unit:
The work unit exists to provide skilled nursing support and services for disabled individuals residing in group homes scattered throughout the Denver Metro area (mainly Jefferson County). The Wheat Ridge Regional Center (WRRC) exists to serve individuals who qualify for active treatment services as defined in Intermediate Care Facilities rules and regulations for individuals with Intellectual Disabilities (ICF-IID). The nursing unit exists to provide 24/7 services to these individuals who have profound and severe disabilities with a high level of medical needs, and/or have significant behavioral and psychiatric challenges. The focus of treatment is stabilization, increasing knowledge base, increasing knowledge of treatment options (i.e., medications), individual and group therapy, discharge planning, working with guardians, family members, and other support systems for individuals. Understanding of health-related and medical issues in individuals with intellectual and developmental disabilities. Individuals with developmental disabilities, physical handicaps, and multiple medical and psychiatric diagnoses require multiple needs in addition to mental health treatment and stabilization. Individuals are admitted for evaluation, observation, treatment, and stabilization. The goal is to return them to their optimal level of functioning and assist them in returning to the community as appropriate.
Description of Job
Summary of the position:
The purpose of this position is to provide constant and consistent nursing oversight, administrative oversight, and management of nursing personnel 24 hours/day, 7 days a week. This position serves as a direct supervisor and appointing authority over Nursing staff. This position requires carrying a state-issued cell phone while not at work and answering this cell phone immediately when not at work. Provide additional constant/consistent management and direction to all direct care nursing staff and nursing shift supervisors. Serves as a resource and mentor to all nursing staff. Promote and maintain nursing staff efficiency and effectiveness. Oversee the attainment and achievement of Nursing Department goals. Collaborates with other leaders across the organization. Represents the Nursing Department in committees and work groups. Applications of knowledge, expertise, and experience in providing safe and effective nursing care. Provide immediate crisis intervention and response to all emergency situations.
This position is responsible for the administrative/management of nursing staff, clinical direction/supervision of nursing staff, QA-Pl activities to ensure compliance in all areas, staff development/competency and interdisciplinary coordination.
This position is the primary infection prevention coordinator and trainer for the facility and is responsible for reporting and communication with state and local Health Departments. This position is accountable to the Director of Nursing (DON).
This full-time position sits on various committees, facilitates groups, and ensures nursing standards of care are implemented throughout all phases of the nursing process and delivery of services.
The Assistant Director of Nursing works with multiple levels of professional services, as care is coordinated across a continuum. The Assistant Director of Nursing adheres to and references the professional codes of conduct, regulatory agency standards, the Nurse Practice Act, State Board of Nursing, professional codes of ethics, and scope of discipline practice guides to ensure various levels of nursing personnel function within their scope of practice. The Assistant Director of Nursing is responsible for communicating, implementing, and clarifying nursing policies at the home level. Developing and implementing performance improvements at the home level, along with procedures at the home level. Participates in and prepares nursing personnel for onsite surveys, monitors documentation, and ensures quality of care is rendered according to the State Board of Nursing, The Joint Commission, Colorado Department of Public Health and Environment, and the Colorado Department of Personnel Rules and Regulations.
Responsibilities also include the provision of safe, appropriate nursing care in the home by effective deployment of nursing staff resources, ensuring enough staff with licenses on each shift, coordination and timely statistical reporting of incidents, acuity, QA/Pl activities, implementation and maintenance of department policies and standards of care, responsible for education of the nursing staff on their home and recruitment/retention of nursing staff on their home (s). Must be able to meet and perform all essential functions and job duties
Primary duties include:
+ Provides direct supervision and appointing authority over Nursing staff, including performance management planning and evaluation, signing and approving leave, and recommending progressive action to the Appointing Authority.
+ Addresses employee performance issues, monitors employee absences, and determines appropriate corrective action and/or support/coaching opportunities.
+ Meets with Wheat Ridge Regional Center Nursing team weekly for individual supervision to provide guidance, direction, instruction, and mentoring.
+ Directs, monitors, and is responsible for all nursing FTE on all assigned shifts/homes. Covers as acting Director of Nursing when assigned.
+ Assesses staff's level of fatigue and ability to function when working a shift overtime/flex time.
+ Coordinates staffing schedules on all shifts for the nursing department, utilizing knowledge of acuity, vacancies, recruitment, hiring, and pool personnel availability to maximize the quality of care given to individuals served by the nursing department. Ensures schedules are maintained and accurate.
+ Meets with new employees and ensures the new employee is assigned a preceptor.
+ Assures that the nursing staff are receiving clear and congruent information.
+ Ensures that duties are performed & documented correctly to include all aspects of each nursing staff member's job description.
+ Assesses any claims of policy violation and alerts the Director of Nursing and Human Resources as appropriate.
+ Communicates general nursing personnel needs to the Nursing Department and works with the Director of Nursing to initiate and maintain position descriptions, and to retain staff and fill vacancies, via promotion, recruitment of new hires, and internal transfers.
+ Clinical Direction & Infection Prevention
+ Responsible for the Infection Prevention/Control program and training for the facility. Additionally, this position is the point person of communication with state and local Health Departments and acts as the facility's liaison as it pertains to response and responsibility in regard to state and national infection requirements.
+ Utilizes knowledge of patient acuity and nursing practice, as well as balancing nursing personnel.
+ Serves on oral boards/committees as appropriate.
+ Responsible for nursing care (medical and psychiatric) for assigned home when covering or supervising.
+ Implements and monitors to ensure adherence to hospital policies/nursing procedures.
+ Ensures and maintains nursing standards of care.
+ Implements and monitors nursing procedures, forms, resources, guidelines, computerized training, nursing groups, and competencies
+ Implements and monitors education, training, and competency for individuals and staff to include agency and pool nursing staff working in the Nursing Department.
+ Maintains current knowledge of literature related to nursing and to specialty areas.
+ Directs the implementation of interdepartmental activities at Wheat Ridge Regional Center by representing the Nursing Department on committees, building and maintaining active communication between nursing and other disciplines/departments, building and maintaining communication with superiors and assigned nursing personnel, and disseminating information.
+ Ensures nursing staff provide services/treatments per licensure requirements, regulatory agencies, governing bodies, and Nursing Department standards, policies, and procedures.
+ Responsible for ensuring that nursing staff follow the nursing standard of care/Colorado nurse Practice Act or Colorado Psychiatric Technician Practice AcUColorado Licensed Practical Nurse Practice AcUNursing Process.
+ Communicates with other leadership within the team to ensure nursing directives are shared.
+ Collects, directs, documents, and informs nursing staff/physicians of patient condition and response to treatment.
+ Reports abuse allegations immediately to the Director of Nursing. Refer to public safety.
+ Monitors medication variances for patterns.
+ Implement actions to maximize the productivity of nursing personnel assigned to home(s) by modeling and participative leadership.
+ Addresses conflicts between nursing employees, providing staff direction, support and feedback, serving as a resource, and facilitating communication.
+ Assesses and provides ongoing evaluation of staff development or competency needs of all shifts.
+ Assures orientation of new nursing staff.
+ Assigns preceptors and monitors students.
+ Serves as a liaison with staff development.
+ Attends in-service training to meet own educational and mandatory training requirements.
+ Ensure nursing staff know and follow various manuals, standards and regulations to include but not limited to: Pharmacy Formulary, Behavior Plans, Infection Control Care Manual, Centers for Medicare and Medicaid Services, Safety Data Sheets, Colorado State Personnel Procedure Manual, Affirmative Action Policy, Colorado Workers Compensation Guidelines, Emergency Guides, ADA statutes, Individual Rights.
+ Ensure all staff are current in mandatory training, computer-based training, rosters, policies, and procedures.
+ Evaluate and assign nursing staff competency in the delivery of resident care. Provide on-the-spot training as needed.
+ Responsible for providing education/training for all New Employee Orientation (NEO) classes.
+ Assumes 24-hour responsibility for the nursing care provided and maintenance of a safe, secure, and therapeutic environment.
+ Identifies and maintains staff/patient boundaries.
+ Must travel between group homes to provide coverage, supervision, nursing care, and training.
+ Travel between group homes using a State vehicle to transport residents as needed.
+ Anticipates potential problems and responds to correct personnel and environmental deficits by facilitating staff communication related to resident care and by overseeing resident health care needs.
+ Directs, implements, and reviews nursing care provided in the homes for all shifts, verifies compliance with standards of nursing care, reviews with RNs under supervision, conducts independent reviews and audits, has knowledge of accepted standards, leads by example, promotes professionalism, and patient care quality assurance regional center-based meetings.
+ Implement proper procedures for respiratory services, oxygen and oxygen equipment, and laboratory services after hours.
+ Evaluates concerns related to incidents and reporting to supervisors. Intervenes in critical situations as necessary, which may include physically restraining aggressive, assaultive patients, performing CPR, or performing physical interventions.
+ Counsels and educates residents on medications, physical care needs, and psychiatric care needs.
+ Conducts treatment and care plan reviews, communicating and educating individuals, guardians, and their family members.
+ Utilizes clean, aseptic, and sterile techniques when required.
+ Monitors medication/treatments given and ensures competency.
+ Participates and ensures staff are using active and passive individual identifiers prior to administration of medications, treatments, obtaining blood for glucose testing, and starting an invasive procedure, and identifies individuals by a minimum of two methods.

Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC) Four (4) years of professional experience in an occupational field related to the work assigned to the position

Appropriate education will substitute for up to two (2) years of the required experience on a year-for-year basis
