Description:
**Job Title:** Nurse Case Manager
**Ministry/Facility:** Trinity Health PACE- St Joseph
Position Purpose:
The Nurse Case Manager coordinates and evaluates health care and social services for participants in the PACE program. Acting as a liaison between the interdisciplinary team (IDT), participants, caregivers, and community agencies, this role ensures seamless care delivery and support.
What You Will Do:
+ Lead and collaborate with the IDT to develop and implement individualized care plans.
+ Conduct assessments, home visits, and hospital discharge coordination.
+ Maintain accurate documentation and communicate effectively with participants and families.
+ Mentor staff, manage departmental operations, and participate in quality improvement initiatives.
+ Ensure compliance with federal, state, and organizational regulations.
Minimum Qualifications:
+ RN license in the state of the PACE organization; BSN preferred.
+ CPR certification required.
+ 2+ years of experience in geriatric care; 1+ year working with frail or elderly populations.
+ Valid driver’s license and insurance.
+ Strong clinical, leadership, and communication skills.
