    Job Details

    The Social Worker is a member of an interdisciplinary team responsible for assessment, treatment and the provision of direct treatment services to mental health and/or chemical dependency behavioral health patients utilizing a variety of clinical social work treatment techniques and modalities. Adheres to Mercy Hospital/department policies and procedures and JCAHO/regulatory agency requirements. Demonstrates and promotes service excellence at all times.

    Responsibilities:

    + Provides a full range of Social Work Services to patients from admission to discharge.

    + Participates in the interdisciplinary team care process.

    + Interacts effectively with external agencies in regard to discharge planning issues.

    + Advocates for the optimal discharge plan for 3K patients.

    + Leads therapeutic groups for patients.

    + Provides clear and comprehensive documentation.

    + Communicates effectively and with the interdisciplinary team.

    + Is knowledgeable about the intricacies of local discharge planning options.

    + Maintains productive relationships with patients.

    + Is self-directed and able to work independently as needed.

    Qualifications:

    + Masters Degree in Social Work from an accredited college/university

    + Licensed in NYS as an LMSW/LCSW

    + Two years experience in a social service agency preferred.

    + Psychiatric social work experience preferred. Knowledge of community resources.

     

    Salary Range

     

    USD $39.92 - USD $46.00 /Hr.
