Nature of Work
Performs a variety of direct client services, case management, and related support tasks for certain at-risk populations.
Environmental Factors
Work is performed primarily indoors in various facilities, which may include homes, and other onsite locations; travel may be required with exposure to all weather and traffic conditions.
Physical Factors
Light physical demands; some lifting and moving of supplies and equipment.
Work Situation Factors
Position involves competing demands, performing multiple tasks, working to deadlines, frequent work at night, beyond normal business hours. Regular attendance is an essential function of this job to ensure continuity of services. Position is subject to drug testing in accordance with applicable State and Federal regulations and City of Las Cruces policies.

This recruitment will be used to fill a full-time regular, exempt position within Quality of Life, but may be used to fill other full time Case Manager positions in other City Departments.
Full-time, Regular, Exempt position.
This position is graded RX13.
Duties and Responsibilities
+ Conducts interviews at various locations and facilities that may include clients' homes, to determine and provide services that protect the health and safety of clients, in compliance with state and federal regulations; determine program and client eligibility for services needed; assigns services and monitors clients progress on a regular basis to effectively manage caseloads; develops and implements client care plans.
+ Maintains regular communication with staff, family members, and professional agencies to access additional community services; conducts field visits and visual assessments of client’s physical surroundings to assess hazards to health and safety; oversees the management of client services of various subcontracting agencies to ensure compliance with federal and state regulations and stipulations of the contracts, grants, and agreements.
+ Maintains all client documentation, including written assessment tools, case notes, family correspondence, referrals, legal reports, and documents; maintains and updates records in various mediums and formats to provide accurate and timely information and reports and tracking to ensure assessments and services are arranged and provided; coordinates information and referrals with other social service agencies and health care providers to obtain necessary services and protect health and safety of clients.
+ Maintains on-going community networking efforts with various agencies, including but not limited to health care agencies, schools, hospitals, doctors, and public and private social service agencies to ensure appropriate services are provided to clients; develops and coordinates special projects that may include in-home services, planning, promoting, implementing, and evaluating special activities and events.
+ Participates and assists in special events, activities, programs, and behavioral management services, and develops and coordinates training.
Minimum Qualifications
Bachelor’s degree in Social Work (BSW) or related field AND two (2) years of experience in casework management, or community population health service programs is required. Experience working with youth, community organizations, volunteers, or other service providers may be preferred depending on assignment. A combination of education, experience, and training may be applied in accordance with City of Las Cruces policy.
Licenses/Certification(s)
Valid driver’s license is required. Position requires an acceptable driving record in accordance with City of Las Cruces policy. Bilingual (English / Spanish) skills may be preferred. Case Management Certification and/or Master’s Level Social Worker (MSW) may be preferred.
Knowledge, Skills, and Abilities
Knowledge of: Geriatric or elder care issues and services when working with elderly population, and high risk clients; youth and family care issues and services; available programs and social services in the community; case management process and Long Term Care services including, but not limited to, eligibility for State and Federal benefits and insurance, DD Waiver, D & E Waiver, SSI and SSDI, Medicare Parts A and B, Medicaid, Medically Fragile, Durable Medical Equipment, PC Option, and other social or community benefits; policies, procedures, and regulations governing eligibility for clients; effective customer service, communication, and problem resolution techniques and methods; occupational and environmental safety and health hazards, and safety practices and procedures; effective interview techniques and procedures; principles of record keeping and records management; business and personal computers, and standard software applications; City organization, operations, policies and procedures to effectively perform the required functions and duties of the position.
Ability to: perform a variety of duties and responsibilities and assess and prioritize multiple tasks, projects, and demands to meet deadlines; ensure appropriate levels of customer service to achieve expectations and meet objectives; read, understand, interpret, and assure compliance with a variety of policies, procedures, rules, and regulations governing related activities, programs, and functions; research and compile applicable information and maintain accurate records; create and maintain effective treatment plans; maintain confidentiality of sensitive information and data; communicate effectively in verbal and written forms; prepare and present accurate and reliable reports containing findings and recommendations; review documents and extract relevant information; establish and maintain effective and appropriate working relationships with employees, other agencies, and the public; operate standard office equipment and timely and accurately enter data using a personal computer with basic and specialized software applications; take initiative and exercise sound independent judgment within established procedural guidelines and carry out primary responsibilities in accordance with all governing regulations, statutes, and procedures to ensure achievement of goals and objectives.
Skills in: communicating and interpreting social service programs and regulations; active listening, providing assessments, and creating treatment plans; analyzing issues, evaluating alternatives, and making logical recommendations based on findings to address issues, improve services, policies and procedures; communicating effectively with employees, clients, and the public; reviewing, evaluating, and verifying records and account documentation; updating information in various mediums and formats to maintain accurate records; communicating information for diverse audiences in a clear and concise manner; working independently and using initiative; reading and interpreting rules and regulations; operating standard office equipment and a personal computer with installed generic and specialized software.
