    Bilingual (Mandarin) Case Coordinator – Adult Day Health

    **Location:** Aging Well Adult Day Care

    420 Maple Street #25, Marlborough, MA 01752

    **Pay:** $23–$25 per hour (based on experience)

    **Schedule:** Monday–Friday, Day Shift (Full-Time, 40 Hours)

    No weekends, no evenings, no on-call

    About the Role

     

    We are seeking a **Bilingual (Mandarin) Case Coordinator** to join our team at Aging Well Adult Day Care.

    As a Case Coordinator, you will provide **organizational assistance and program support** , including admissions, care coordination, and participant case management.

    Key Responsibilities

    + Develop individual care plans to meet each participant’s needs and goals.

    + Assist participants and families in understanding program goals and available services.

    + Serve as a **liaison** between participants, families, and community resources.

    + Organize and chair participant care conferences.

    + Review participant reports (evaluations, progress notes, discharge plans) for accuracy and coordination.

    + Coordinate and document internal and external healthcare appointments.

    + Manage discharge planning and follow-up processes.

    + Review monthly billing reports and communicate discrepancies.

    + Support internal communication among care team members.

    + Attend required meetings, including clinical and supervision sessions.

    + Train new staff on case coordination and medical care oversight as needed.

    Qualifications

    + **Bachelor’s Degree** in a Social Services–related field (required).

    + **Bilingual in Mandarin and English** (required).

    + Minimum **2 years of experience** providing direct or indirect support in a medical, rehabilitation, or disability services setting.

    + Strong organizational, time management, and communication skills.

    + Compassionate, reliable, and team-oriented approach.
