    Title: Community Habilitation Worker

     

    Classification: Full or Part-Time

    Hours: 20-30 hours per week; benefits eligible

    Pay Rate: $15.50/hr (flexible days, evenings and some weekend hours)

    Summary

    Under the direct supervision of the Disability Service Program Manager, the Community Habilitation Worker provides education, instruction, direct services, and support. The Community Habilitation Worker promotes consumer choice and individualized outcomes that empower consumers to achieve their goals and live as optimally as possible. The Community Habilitation Worker and the consumer work together to achieve the goals stated in each individual’s plan.

     

    Essential Job Duties

     

    + Collaborate in creating individualized Community Habilitation Plans for each consumer, tailoring goals to their strengths and needs.

    + Deliver services prioritizing each consumer’s unique needs, preferences, and goals.

    + Actively engage with consumers to address and work toward the specific goals outlined in their individual plans, monitoring progress regularly.

    + Serve as an advocate for consumers, promoting their right to make choices and encouraging them to self-advocate and express their preferences.

    + Assist consumers with daily activities, such as meal preparation and mobility, by providing support only when necessary to foster their independence and confidence.

    + Support the development of social skills through structured education, training, and role modeling to enhance interpersonal interactions.

    + Help consumers participate in community activities and build relationships by guiding them in social settings and introducing them to new experiences.

    + Assist with essential financial tasks, such as budgeting, paying bills, and shopping, to promote financial independence.

    + Attend meetings and Individual Service Plan (ISP) reviews for consumers, contributing to discussions on progress and goal adjustments.

    + Create and maintain a weekly schedule of consumer appointments, ensuring that all required documentation is completed accurately and on time.

    + Follow established safety protocols, conduct regular well-being checks, and respond promptly to potential safety risks to maintain consumer safety.

    + Transport consumers as outlined in their service plans, accompanying them to appointments, community activities, and errands, and remaining with them at all times to ensure their safety.

     

    Additional Job Duties

     

    + Attend all scheduled program, training, and staff meetings.

    + Other related job duties as assigned.

     

    Knowledge, Skill and Abilities

     

    + Demonstrate the ability to establish and maintain professional and respectful relationships with consumers, team members, and external agency partners.

    + Demonstrated cultural competence and ability to interact effectively with consumers and team members of diverse cultural, racial, and ethnic backgrounds.

    + Strong record-keeping and reporting skills, with exceptional attention to detail.

    + Excellent communication skills, oral and written.

    + Maintain general knowledge of community resources and ability to connect consumers with relevant services to support their individual needs.

    + Familiarity with computers and Microsoft applications such as Outlook, Word, and Excel.

    + Flexibility to work occasional evening and weekend hours as required to meet consumer needs and program requirements.

    + Must be able to physically perform stooping, kneeling, walking, lifting, grasping, pulling, standing, talking, hearing.

    + Must be able to perform light work exerting up to pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.

    Qualifications

    + Preferred: High school diploma or equivalent and one (1) year of experience working in a related field.

    + Required: Valid NYS driver’s license, reliable transportation, and ability to pass a driving background check.

    + Required: Complete an Employee Criminal History review (fingerprinting) and training mandated by the NYS Office of Children and Family Services (OCFS).
