    PRN Physical Therapist

     

    Schedule: PRN (As Needed)

     

    Join our compassionate, collaborative rehabilitation team where your expertise as a Physical Therapist (PT) helps residents achieve their highest level of function, mobility, and independence. We’re seeking a motivated and licensed PT who is dedicated to providing quality, evidence-based care in accordance with professional standards and facility guidelines.

    Required Qualifications

    + Current, valid, and unrestricted Physical Therapy license in the state of Nebraska.

    + Must meet all educational requirements as defined by the state licensing board.

    + Valid driver’s license in the state of employment.

    + Working knowledge of skilled nursing facility regulations, procedures, and best practices in long-term care.

    Key Responsibilities

    + Plan, develop, and deliver individualized physical therapy programs in accordance with physician orders, state/federal regulations, and APTA standards of practice.

    + Evaluate residents to determine functional abilities, establish treatment goals, and document progress and outcomes per facility and regulatory guidelines.

    + Collaborate with the interdisciplinary team to ensure effective care planning, discharge readiness, and optimal resident outcomes.

    + Monitor and report key performance indicators related to therapy outcomes, regulatory compliance, and resident satisfaction.

    + Maintain accurate documentation, billing, and coding for all services in compliance with facility and payer policies.

    + Ensure therapy equipment is properly cleaned, maintained, and safely used in accordance with infection control and safety standards.

    + Provide clinical supervision and performance feedback to Physical Therapy Assistants in alignment with practice act requirements.

    + Participate in team meetings and committees, fostering communication and collaboration across departments.

    + Promote a positive, respectful, and supportive environment for residents, families, and staff.
