    Exempt, Full-Time (40 hours/week)

    Position Summary:

    The Director of Admissions and Case Management is responsible for coordinating the resident admission process while providing ongoing case management services that promote residents’ social, emotional, and mental well-being. This position serves as a key liaison between residents, families, referral sources, and internal departments to ensure a seamless transition into care and continued high-quality, person-centered support throughout each resident’s stay.

    Qualifications:

    + Bachelor’s degree in Human Services, Social Work, Healthcare Administration, or related field required

    + Minimum of two years’ experience in admissions, case management, or healthcare marketing required

    + Strong interpersonal, communication, and organizational skills

    + Ability to manage multiple priorities and exercise sound judgment

    + Experience working with older adults and families in healthcare or residential settings

    + Knowledge of community resources, social service programs, and assisted living regulatory requirements

    Salary:

    + Starting at $60,405.80, commensurate with experience

    Essential Job Responsibilities:

    + Manage the full admissions process from application to move-in, ensuring timely communication, accurate documentation, and regulatory compliance.

    + Conduct pre-admission assessments to determine eligibility, appropriate placement, and readiness for transition.

    + Serve as primary liaison with hospitals, referral partners, families, and community agencies to facilitate smooth referrals and admissions.

    + Collaborate with Marketing and Administration to support outreach, census growth, and relationship-building initiatives that strengthen community awareness.

    + Coordinate interdisciplinary care planning by partnering with nursing, therapy, dietary, and finance departments to promote continuity and quality of care.

    + Provide case management services, including social assessments, counseling, and assistance with accessing benefits and community resources.

    + Monitor residents’ well-being and coordinate support for social, emotional, and behavioral health needs, including crisis intervention and counseling referrals.

    + Facilitate discharge and aftercare planning to ensure smooth transitions and continuity of support post-discharge.

    + Maintain accurate records and reports for admissions and case management, including resident case notes, census trends, and regulatory documentation.

    + Represent Hultquist Place professionally in community events and referral networks, promoting the facility’s mission, values, and reputation for excellence in resident care.
