JOB SUMMARY:
The Primary Therapist provides quality and organized psychotherapy and social services for identified clients and families, with special emphasis on treatment and discharge planning needs. He/She maintains accurate and up-to-date knowledge of all relevant regulations and standards regarding the provision of social services and of the implementation of treatment and discharge planning. The Primary Therapist assures the collection and evaluation of social services and discharge planning data according to UltraGroup, LLC’s policy, demonstrates professional behavior, and supports the philosophy of Jennie Stuart Health.
Job Knowledge:
+ Knowledge of community resources for older persons, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care. Familiarity with medical terminology and conditions as these impact treatment and discharge needs. Familiarity with medical team roles and with the functioning of other hospital departments. Knowledge of effective individual and group communication techniques.
Essential Job Functions:
+ Interviews clients and families or significant others to obtain social history and data pertaining to treatment and discharge planning.
+ Interprets variables of social situations as these relate to client’s condition, admission and post-discharge needs; modifies social services plan accordingly.
+ Meets in individual or group sessions with clients, families, or both, as requested by Medical Director, physician, or Treatment Team, or to address issues identified in the Social History that affect treatment and discharge.
+ Participates in Treatment Team meetings.
+ Conducts group therapy as requested.
+ Conducts support groups as requested.
+ Works as the Discharge Planner with the Treatment Team to assure provision of appropriate social services to clients.
+ Represents the program at educational meetings as requested by Program Manager.
+ Functions as the point person for Discharge Planning by accurately completing all forms and documents related to discharge, as necessary with input from all members of the staff (primarily the RN and Unit Clerk).
+ Works cooperatively with the Unit Manager/Unit Director/Clinical Director in ensuring that all relevant regulations and standards are met, including HFAP, Medicare, and state licensure. requirements.
+ Collects and compiles data and completes reports as requested by the hospital, Medical Director, and UltraGroup, and as required by the unit’s Quality Improvement Plan.
+ Assists in the monitoring and evaluation of psychotherapy, social services, and discharge planning as specified in the Quality Improvement Plan.
+ Collects ongoing data results for cognitive and effective testing as requested by the Medical Director.
Required Skills
Minimum Experience:
Master’s degree in Social Work from an accredited Graduate School of Social Work.
Required Licensure/Certifications:
+ Has a current licensure such as LCSW, LCPC or LMFT.
+ Must meet any applicable state certification standards.
+ BLS certification - American Heart Association (Healthcare Provider) or American Red Cross (Healthcare Provider) required, must be current.
Required Experience
Minimum Work Experience:
• Minimum of one year clinical experience, preferred in an acute-care setting. Demonstrated understanding of discharge planning, social work concepts, and continuous performance improvement focus preferred. Demonstrated excellence in customer service and communication skills.
Required Licenses: Social Workers
• LSW in the state of KY preferred.
• CCM preferred but not required.
