    Job Title: Case Manager

    Job Description

    We are seeking a dedicated and experienced Care Manager, either a Licensed Clinical Social Worker (LCSW) or a Registered Nurse (RN), to join our team. This role is crucial for supporting hospital care management and discharge planning. The ideal candidate will play a key role in coordinating patient care, facilitating safe discharges, and ensuring continuity of services across various care settings.

    Responsibilities

    + Conduct initial evaluations and develop individualized plans of care.

    + Coordinate discharge planning, including arranging home care, equipment, and follow-up services.

    + Collaborate with internal and external care teams to ensure seamless transitions.

    + Educate patients and caregivers to support discharge readiness and long-term care goals.

    + Document care delivery, patient response, and teaching activities in the Epic charting system.

    + Refer patients to appropriate community resources, rehabilitation, and long-term care facilities.

    + Participate in care conferences and interdisciplinary planning.

    Essential Skills

    + Active LCSW license or RN license.

    + BLS certification through the American Heart Association.

    + 2+ years of hospital case management and discharge planning experience.

    + Proficiency in the Epic charting system.

    + Strong knowledge of discharge planning, care coordination, and community-based services.

    + Familiarity with major disease processes including CHF, COPD, dementia, diabetes, and psychiatric disorders.

    + Excellent communication and interpersonal skills.

     

    Additional Skills & Qualifications

     

    + Previous charge experience preferred.

    + Experience in teaching or community hospitals, acute care, or trauma centers preferred.

    + Experience with protective services, hospice, and long-term care coordination preferred.

    + Must have demonstrated regular and consistent attendance at work as reported by past employer.

    Work Environment

    The position is based at a medical center, operating as a Short Term Acute Care and Level IV Trauma Center. The facility has 60 staffed beds and maintains a daily census of approximately 40 patients. The work schedule is Monday to Friday, 9:00 AM to 5:30 PM, with a requirement to work two weekend shifts every pay period. There is potential to float one day per week to another location. Scrub color is flexible, allowing any color or business casual attire. Free parking is available for staff. The position is an 18-week contract with the possibility of extension, and the work environment emphasizes high-quality, compassionate health care for all.

    This is a Contract position based out of Newberg, Oregon.

    Pay and Benefits

    The pay range for this position is $40.00 - $55.00/hr.
