Job description: RN - Weekend Hospital to Home Private Duty Transition Registered Nurse Shift: Part/Full time Day shift (both Saturday & Sunday 12 hour shifts) per week  Pay Range: $38-$45 We are seeking a compassionate and skilled Registered Nurse (RN) to support the transition of patients from the hospital setting to home care. This part to full-time weekend position involves providing direct patient care, training and supporting families in the home environment, and ensuring a smooth and safe transition process.  Key Responsibilities: •              Provide skilled nursing care to patients in their home during the initial transition from hospital to home. •              Educate and support the patient’s family/caregivers in managing the child’s medical needs and equipment. •              Administer medications and treatments as prescribed. •              Operate and troubleshoot home medical equipment including ventilators, tracheostomy care, and gastrostomy (G-tube) feedings. •              Monitor the patient’s condition and respond to changes or emergencies appropriately. •              Maintain accurate and up-to-date clinical documentation in EMR system.
