    Summary This Social Worker (Program Coordinator) position is in the VA Boston Healthcare System services, located at the Brockton, Massachusetts location. This position is a full-time at 40 hours per week Monday through Friday. Responsibilities Veteran's Health Administration/ Boston Healthcare System (VHABHS) is a Complex Level 1A Facility and provides extensive primary, secondary, tertiary, and quaternary care in outpatient, inpatient, residential, and community settings. VHABHS is a multi-site medical system that includes a major medical facility in West Roxbury, Massachusetts, outpatient clinics and Huntington House in Jamaica Plain, Massachusetts, residential treatment programs in Brockton, Massachusetts, as well as and five Community Based Outpatient Clinics. VHABHS is an academic medical facility with significant teaching and research programs. The CNH Program Coordinator is assigned to the Geriatrics & Extended Care Service line. Duties: Oversees the work of an interdisciplinary staff assigned to VA Boston's Community Nursing Home program, which is one of the largest CNH programs in the nation Provides some direct clinical services through visitation to community nursing home residents and conferences with facility staff; the purpose of these visits is to address and resolve patient concerns as they develop and to ensure that quality services are provided with attention to the dignity of each Veteran and with and age-related capacity in understanding treatments. Responsible for providing leadership, direction, orientation, coaching, in-service training, staff development and continuing education programs for assigned staff. Oversees program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing, and implementing ideas for improvement and evaluating efficacy of improvement efforts. Performs work related to the review, monitoring, analysis, planning and assessment of various programs. S/he is responsible for key project activities related to the planning, development, and implementation of the project component. Collaborates and gains the support of employees for developing, implementing, and managing policies and establishing goals, in addition to working to achieve the objectives of government wide personnel programs and policies. Manages and oversees outcome data for all programs assuring it is collected, analyzed, and appropriate actions plans are set in place. (This includes collecting and analyzing stakeholder satisfaction feedback from a variety of sources including patients, family members, and referral sources.) Collaborates with informatics experts in the development and implementation of systems to track services and activities, including visits, referrals, inpatient days, patient demographics, equipment utilization, patient satisfaction, etc. Responsible for overseeing the schedule of required monthly follow-up visits to Veterans in the community nursing homes are completed in a consistent, timely and thorough manner. Assures that annual VA CNH and program reviews are completed as set forth in VA CNH program guidelines, and s/he must have a good knowledge of Centers for Medicare/Medicaid standards regarding CNH facilities. Serves as the primary/lead Contracting Officer Representative (COR) for the CNH program. The incumbent is responsible for assuring that quality assurance related to these programs are within program parameters. Responsible for ensuring the reporting and investigative process for sentinel/adverse events by program staff is completed as necessary. Responsible for actively participating in the Non-Institutional Care Oversight Committee. Maintains a current census list of active participants in the program to enable tracking of service activities, utilization of services and other functions. Interprets central office directives and facilitates the spread of best practices between all VA medical centers Work Schedule: Time varies (Monday through Friday). Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 4 accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs. (a) Skill in coordinating and implementing one or more specialty program(s), across the facility. (b) Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive. (c) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (d) Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. (e) Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. (f) Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. (g) Knowledge of organizational structure and impact on program operations.
