In this role, you’ll provide support to ensure seamless care coordination, efficient operations, and an exceptional patient experience.
Position Type: Full-time, Day Shift (Monday–Friday, 8:00 a.m.–4:30 p.m.)

What You’ll Be Doing:
The Patient Care Navigator serves as a key point of contact between patients, providers, and facilities—helping coordinate care, manage documentation, and support daily clinical and administrative operations.
+ Answer phones and route communications appropriately, schedule appointments, and document interactions in the EMR system (Medent/PCC).
+ Complete chart preparation for specialty appointments and coordinate schedules for providers across Assisted Living and Skilled Nursing Facilities.
+ Complete prior authorizations for medications and imaging services to ensure timely care.
+ Review and process faxes, medical record requests, and triage items throughout the day.
+ Send Transition of Care (TOC) discharge summaries to appropriate departments, including GLIN nursing staff.
+ Maintain a safe, secure, and compliant work environment by following established policies and procedures.
+ Deliver exceptional customer service to provide a positive and supportive patient experience.
+ Manage incoming document bins, assist with the completion of forms, and support the team as needed.
+ Work independently and collaboratively to ensure efficient operations and continuity of care.
Additional Responsibilities:
+ Provide coverage and assistance when teammates are out.
+ Participate in staff meetings and one-on-one sessions.
+ Perform other duties as assigned by management.
The Education and Experience You Need:
+ Education: High school diploma required; certification or degree in a healthcare-related field preferred.
+ Experience: Minimum of 3 years of experience in a healthcare setting preferred.
+ Knowledge: Familiarity with Medent EMR and healthcare documentation preferred.
+ Skills: Strong written and verbal communication skills, attention to detail, and proficiency with computer systems.
