The Work Unit :
The Prevention Services Division (PSD)is actively building a workforce reflective of the communities we serve. The Health Promotion and Chronic Disease Prevention (HPCDP) Branchin Prevention Services Division (PSD), reviews, plans, designs, implements and evaluates public health chronic disease interventions and related risk factors with the goal of advancing health access and improving health-related quality of life for all Coloradans. Special emphasis is placed on evidence-based practices, data-based and evidence-informed decision making, capacity-building, partnerships, and integration between program efforts for efficiency and effectiveness. The statewide chronic disease programs within the HPCDP Branch include Heart Disease and Stroke Prevention, Comprehensive Cancer Control, Colorectal Cancer Control, Diabetes Prevention and Management, Alzheimer’s and Related Dementias (ADRD), Healthy Eating and Active Living, the Cancer, Cardiovascular and Pulmonary Disease Grants Program and the Tobacco Education, Prevention and Cessation Grants Program.
We are aware that many chronic diseases share the same systemic causes including tobacco use; lack of access to nutritious foods and built environments to support physical activity; and social connectedness, among others..
Our work will continue until all Coloradans have the opportunity to live and have healthy lives and i we work to achieve this by decreasing the burden of disease, supporting health-related social needs, and growing health care access across Colorado through all of our programs and practices.
This position is housed in the Diabetes, Cardiovascular Health and Alzheimer’s Disease and Related Dementias Unit (CarDiADRD Unit) which focuses on engaging with health systems, community-based organizations and other partners to improve chronic disease outcomes for all Coloradans. The unit also collaborates with internal programs including those focused on cancer, healthy eating and active living, tobacco, informatics, and others, to support a comprehensive system of partnerships that address specific chronic disease needs in innovative and sustainable ways.
The Position:
This position leads the unit’s payment reform and sustainability projects and activities within the unit’s determined strategy. This strategy focuses on the integration of chronic disease prevention and management programming in health systems through the leveraging of value-based payment models, quality improvement partnerships and policy and reimbursement opportunities. This position serves as the subject matter lead on payment reform and sustainability strategies and provides technical support to partners on best methods of achieving chronic disease prevention and management program sustainability goals. This position informs and implements project strategy, driving action that leads to effective and advantageous communications, partnerships and integration.
This position works with CarDiADRD program managers to identify ways to include prevention and management efforts into value-based payment opportunities and will represent the unit’s voice in ongoing payment reform and coverage conversations with external partners, including participating in the Primary Care Payment Collaborative and other similar efforts as they arise.
This position informs and implements the unit’s sustainability plans per direction of program managers. This includes engaging as a partner in conversations with other state agencies, collaboratives and partnerships designing or implementing alternative payment models such as Medicaid’s Regional Accountable Entities. It includes engaging with partners on ways to increase quality and sustainability of work conducted by ancillary providers (pharmacists and community health workers, among others) that increases access to chronic disease prevention and management interventions such as the National Diabetes Prevention Program, Diabetes Self-Management Education and Support, medication optimization, self-measured blood pressure monitoring or others. This work is evolving and requires this position to be nimble and innovative within the parameters set by the supervisor.

MINIMUM QUALIFICATIONS:
An applicant may meet the minimum qualifications for this job in one of two ways: They may qualify through their work experience alone or through a combination of education and work experience.
Experience Only:
Six (6) years of relevant experience in health policy, accountable care, health care payment strategies and/or models, health care network administration, evidence-based disease management and/or prevention in public health or health-related organization.
OR
Education and Experience:
A combination of related education, graduation from an accredited college or university in public health, health education/promotion, health policy, health care administration, pharmacology, social work, or other health-related fields of study,and/or relevant experience in health policy, accountable care, health care payment strategies and/or models, health care network administration, evidence-based disease management and/or prevention in public health or health-related organization equal to Six (6) years.

PREFERRED QUALIFICATIONS
In addition to the minimum requirements, the successful candidate in this position will have the following experience:
+ Experience in or understanding of self-measured blood pressure monitoring programs, cholesterol management approaches, and/or clinic-based quality improvement programs geared toward enhancing cardiovascular prevention and management;
+ Experience in engaging clinical and community providers, including physicians, pharmacists, nurses, community health workers as well as community-based health program providers;
+ Experience in diabetes and cardiovascular disease prevention management
+ Experience managing coalitions and facilitating meetings with stakeholders;
+ Demonstrated ability to establish collaborative partnerships and effective working relationships with a variety of internal and external customers and diverse stakeholders;
+ Experience working with and demonstrated understanding of health systems, accountable care organizations, provider networks, and payers, including health plans and employers;
+ Excellent skills in project management and negotiation, including establishing goals and timetables, securing and coordinating resources, monitoring progress, engaging and working with other personnel or teams, measuring results, developing contingencies, and reporting;
+ Outstanding skills and abilities in verbal and written communications; ability to adapt a style to fit diverse audiences of varying technical levels, and at various levels within an organization, in groups and individual settings;
+ Initiative & self-management; the passion and ability to dive into the work and accomplish tasks within established timeframes; and
+ Experience and demonstrated skill in community engagement practices, particularly as applied to working alongside communities experiencing high burden of chronic diseases and/or their systemic causes.
+ Experience translating complex concepts into concrete strategies.
CONDITIONS OF EMPLOYMENT
+ Pursuant to CDPHE policy, a background check is required for all positions at CDPHE.
+ TRAVEL: 10% of time, required to travel in-state to meet with local stakeholders and partners, with occasional out-of-state travel to meet with national partners. Local travel is approximately 2-3 times monthly; out-of-state travel is approximately twice per year.
