    About Forensic Based Community Services

     

    Located within the ColoradoDepartment of Human Services, the Office of Civil and Forensic Mental Health (OCFMH) is responsible for policy development, service provision and coordination, program monitoring and evaluation, and administrative oversight for the public behavioral health system . OCFMH funds, supports and monitors numerous mental health and substance abuse community programs and providers . OCFMH also operates the Colorado Mental Health Hospitals at Fort Logan and Pueblo and the Forensic Services Division . OCFMH executes the States' federal responsibilities as the State Mental Health Authority and State Substance Abuse Authority for the purposes of administering federal mental health and substance abuse block grant funds .

     

    The Forensic Services Division is responsible for six departments including Forensic Community Based Services (FCBS), Jail Based Evaluation and Restoration (JBR), Outpatient Restoration (OPR), the Forensic Support Team (FST), theCourt Services Department, and the Operations Department. The Court Services Department employs forensically practicing psychologists to provide evaluative services statewide, with staff in Pueblo , Denver, and other locations . Evaluators complete all court ordered forensic evaluations for defendants in criminal cases as ordered by the court . These evaluations are performed in the jail, in the community, at CMHHIP, or at CMHHIFL and are performed by psychologists and psychiatrists, as mandated in the Colorado Statute, 16-8 . 5 .

     

    This work unit exists to carry out the statutory obligations of the Department of Human Services to supervise compliance with court-ordered conditions for individuals who have been adjudicated Not Guilty byReason ofInsanity (NGRI)and who are subsequently relased by the courts from their commitment to CDHS (Conditional Release) and at times when they are granted extended leave from the hospital into the community, Temporary Physical Removal (Conditional Placement) . Additionally, the work unit provides oversight care coordination, and in select cases treatment interventions, for individuals who have been found NGRI orin competent to proceed and who remain committed to the custody of CDHS, but who have been released by the courts to reside in the community .

     

    Our teams work within various settings including community agencies, jails , courts, and hospitals. The locations can include locked units and secure facilities and our clients may at times be unpredictable due to their mental health needs. Our team is required to make critical decisions in dynamic and fast-paced environments in order to ensure a high level of accountability for patient care . Our staff are required to maintain updated skills, knowledge , abilities, and competencies necessary to the position through continuing education,clinical supervision, literature review, and discipline and team development. This position may be required to travel throughout Colorado to meet the needs of the department.

    Description of Job

    About this Position:

    The Health Professional V (HPV) serves as a supervisor within the department to various other staff and offers internal and external consultation regarding the Not Guilty by Reason of Insanity (NGRI) population, Statute, and FCBS process and procedure. The HPV offers supervision and guidance, ensuring staff are adhering to legal obligations, internal policy and procedure, best practices, and client needs. The position is responsible for personnel action matters including performance reviews, performance plans, corrective actions, and training. They are a primary liaison with internal and external stakeholders to include the mental health hospitals, community providers, courts and attorneys, and client support systems. The HPV maintains a caseload of FCBS clients as determined by workload needs and client acuity and determines the caseload distribution of supervisees in partnership with other team members. As such, this position is responsible for contributing to care coordination and case management needs of clients with a variety of presentations and resources. In doing so, this position must maintain strong knowledge of the Colorado statutes, the identified person’s risk and mitigation factors, community resources, and complex system processes. The HPV is also responsible for ensuring legal obligations are adhered to on behalf of the department, which may include enforcing conditions of community living, reporting to the court, identifying violations, drafting conditions, offering court testimony, partnering with providers to identify psychiatric decompensation, and coordination of any interventions. The overall goal of this work unit and this position is to assist with smooth and safe transitions into the community, support maintenance of stability in the community, support ongoing community treatment engagement, and support client’s interactions with the legal system.

    Job Duties:

    Care Coordination and Oversight:

    + Responsible for a caseload of FCBS clients as determined by referral demand and acuity, as well as assigning caseloads to supervisees. This begins at the receipt of referrals for community living and the commencement of outreach efforts with the hospital teams to begin establishing rapport and gaining knowledge of individualized needs.

    + Monitors the ongoing psychiatric stability, safety, and support needs for those living in the community under specific agreed upon and/or court ordered conditions

    + Monitors and supports adherence to any agreements and/or court ordered conditions, and communicates timely with necessary parties if there is a change or violation

    + Serves as a primary liaison and consultant with community stakeholders

    + Supports any resource referral needs in partnership with treatment team members, engages client support networks, and offers case management service needs

    + Attends staffings, may participate in home visits, and facilitates administration, collection, and submission of various forensic tests (i.e., urinalysis, hair test, etc.). Utilizes test results in order to inform care needs

    + Contributes to plans of care when appropriate, monitors condition compliance (i.e., housing, employment, medication, treatments, etc.)

    + Monitors ancillary individual needs such as adequate finances, food, clothing, transportation, and property and home searches

    + Partners with treatment teams to facilitate referrals for evaluations and changes in conditions

    + Drafts corresponding documentation and enters regular contact notes in various electronic health record platforms, in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

    Supervision and Personnel:

    + Responsible for employee supervisory responsibilities within the work unit, which includes a minimum of three (3) FCBS staff

    + Oversees a variety of staff, which may include case managers, specialists, and administrative staff

    + Provides oversight to their associated processes, documentation, and legal submissions in partnership with the director

    + Supervisory duties include but may not be limited to individual supervision, group supervision, training, onboarding, professional development, staff scheduling, position descriptions, recruitment, retention, interviewing and hiring, ensurance of competencies, documentation review, performance reviews, performance plans, corrective actions, consultation, and guidance to ensure all supervisees are functioning within statute, policy, and procedure

    + May participate in various department trainings, contribute to such training, review statute and offer suggestions, and engage in any auditing activities

    Documentation:

    + Required to regularly document client contact notes within theinternalelectronic health record system in a timely, thorough, and accurate manner

    + Responsible for legal documentation to be submitted to the courts which includes but is not limited to quarterly reports, annual reports, probable cause memorandum for violations and assist with drafting probable cause affidavits , draft "RequirementsforCommunity Placement", draft conditional release orders, and other FCBS related court communications and/or reports

    + C ompleting all documentation professional writing, attention to detail and relevant information, and an ability to accurately convey information is expected

    + Directly submitting such documentation, this position is responsible for the review/approval of various documents completed by those they supervise prior to submission to the director for approval

    Legal and Court Involvement:

    + Serves as a primary point of contact for judges, attorneys, guardians, attorneys general, and others affiliated with the criminal justice system. This involves fielding questions and concerns via email, telephone, meetings, and training. This also includes testimony when so required on behalf of the Forensic Services Division

    + Required to contact law enforcement. When engaging in any such activities, this position is required to maintain professional boundaries, scope of practice associated with the position, and represent the department

     

    Other Duties as Assigned

     

    + Any other duties that may arise while completing the tasks of this position to meet the needs of the unit and Department and Division.

     

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

    Minimum Qualifications

    Experience Only:

    Eight (8) years of relevant experience in an occupation related to the work assigned to this position

    OR

    Education and Experience:

    A combination of related education in Social Work, Psychology, Human Services and/or relevant experience in an occupation related to the work assigned equal toeight (8) years.

    Preferred Qualifications

    + A Master’s level license in mental health or related field (LSW, LCSW, LPC, etc.)

    + Master’s degree in mental health or related field

    + Experience with supervision

    + Experience in case management, probation/parole, mental health treatment, or similar

     

    Conditions of Employment

     

    + This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.

    + Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.

    + Must possess a valid, non-restricted Colorado Driver’s License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border

    + Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.

    + Must transport patients in state vehicles, and travel to various locations statewide for program needs

    + Pre-Employment Physical-TB Test and otehr vaccination required by the State of Colorado

    + CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check.

    + This position will also provide direct care to vulnerable adults requiring a CAPS check (as defined by C.R.S. 26-3.1-101, position provides services & support care to at-risk/vulnerable adults including case management, clergy, physical or mental health services, safety or welfare services).
